Endoscopic repair of large nasal septal perforation (SP) remains a rather challenging procedure. The presented modification of vascularized flap, which is supplied with branches of posterior septal artery, has proven to be convenient and effective in closure of SPs larger than 2 cm. Among repairs for ventriculoarterial discordance, ventricular septal defect, and pulmonary stenosis, aortic root translocation (Nikaidoh operation) offers the most anatomic result. With a diminutive pulmonary annulus or hypoplastic left ventricular outflow tract, the distance gained posteriorly with aortic translocation is negligible. We developed the "hemi-Nikaidoh" procedure as an alternative.

Currently, the direct endonasal approach is widely used in endoscopic endonasal surgery (EES) for pituitary neuroendocrine tumor. However, a large posterior septal perforation is inevitable. We routinely utilize a modified para/transseptal approach using the combination of a Killian and a contralateral rescue flap incision (PTSA with K-R incision).

Maternally activated connections of the ventral lateral septum reveal input from the posterior intralaminar thalamus.

The lateral septum (LS) demonstrates activation in response to pup exposure in mothers, and its lesions eliminate maternal behaviors suggesting it is part of the maternal brain circuitry. This study shows that the density of pup-activated neurons in the ventral subdivision of the LS (LSv) is nearly equivalent to that in the medial preoptic area (MPOA), the major regulatory site of maternal behavior in rat dams. However, when somatosensory inputs including suckling were not allowed, pup-activation was markedly reduced in the LSv.

Among contributors to diffusible signaling are portal systems which join two capillary beds through connecting veins. Portal systems allow diffusible signals to be transported in high concentrations directly from one capillary bed to the other without dilution in the systemic circulation. Two portal systems have been identified in the brain.

Background: To investigate the optimization of leadless pacemaker placement and to assess its impact on heart synchronization and tricuspid regurgitation.

Results: A clinical trial was conducted involving 53 patients who underwent leadless pacemaker implantation at the Second Affiliated Hospital of Zhejiang University School of Medicine and Hangzhou First People's Hospital between March 2022 and February 2023. Implantation site localization was determined using the 18-segment method under RAO 30° imaging.
