A 14-year-old male patient was successfully treated with the reamer irrigator aspirator for femur intramedullary rod preparation after sustaining right and left closed femur fractures because of an all-terrain vehicle accident. In patients already categorized as high risk for fat embolism syndrome, such as those with bilateral femur fractures, reaming both femora greatly increases the likelihood of this complication. The reamer irrigator aspirator provides an effective tool that potentially mitigates the risk of fat embolism syndrome in pediatric patients with this type of orthopedic trauma.

Between July 2021 and February 2024, 6 patients with biopsy-confirmed benign pathologic lesions had mandibular defects reconstructed using a reamer-irrigator-aspirator at the University of Cincinnati Medical Center. Patients' ages ranged from 34 to 73, 5 of which were males and one female. Primary end points were bony continuity of the mandible, recovery time, and ability of the graft to receive implants.

The Reamer-Irrigator-Aspirator (RIA) device represents a safe and efficient method to harvest autologous bone for grafting. However, hardware failure may occur, for example by breakage of the reamer head with metal debris remaining in the intramedullary canal. This case report describes the uncomplicated secondary removal of femoral intramedullary metal debris from a broken RIA reamer head; three weeks after the final surgery of a two-stage Masquelet procedure for the treatment of posttraumatic segmental bone loss at the tibia.
