Ensuring authorized access control in the IoT is vital for privacy and safety protection. Our study presents the novel IHIBE framework, which combines IOTA (a distributed ledger technology) with hierarchical identity-based encryption (HIBE), thereby enhancing both IoT security and scalability. This approach secures access tokens and policies while reducing the computational demand on data owners. Our empirical findings reveal a significant performance gap, with access rights delegation on the Raspberry Pi 4 exceeding those on AWS by over 250%. Moreover, our analysis uncovers optimal identity policy depths: up to 640 identities on AWS and 640 on the Raspberry Pi 4 for systems with higher tolerable delays, and 320 identities on AWS versus 160 on the Raspberry Pi 4 for systems with lower tolerable delays. The system shows practical viability, exhibiting insignificant operational time differences compared to Zhang et al.'s schemes, particularly in access rights verification processes, with a minimal difference of 33.35%. Our extensive security assessment, encompassing scenarios like encrypted token theft and compromise of authority, affirms the efficacy of our challenge-response and last-word challenge (LWC) mechanisms. This study underscores the importance of platform choice in IoT system architectures and provides insights for deploying efficient, secure, and scalable IoT environments.

Objective: To investigate the application value of arthroscopic channel modification in meniscal injury repair.
Methods: We retrospectively analyzed the data of 100 patients with meniscus injuries treated with knee arthroscopy from December 2022 to December 2023 and divided them into a control group and a modified group according to the application of "arthroscopic access modification technology". We compared the operation time, postoperative hospitalization time, VAS score, Lysholm knee function score, postoperative complications, and postoperative images of the patients in these two groups.

Since there was no previous pooled data regarding the intestinal parasites and Helicobacter pylori co-infections among gastrointestinal symptomatic patients in the African context, this review aimed to determine the overall prevalence of intestinal parasites and Helicobacter pylori co-infection in people with gastrointestinal symptoms in Africa.
Methods: The current review followed the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) standards and registered in the International Prospective Register of Systematic Reviews (PROSPERO; CRD42024598993).

The coronavirus Disease 2019 (COVID-19) pandemic has disproportionately impacted long-term care facilities (LTCFs), revealing vulnerabilities due to residents' advanced age, comorbidities, and facility infrastructures. In Taiwan, the Central Epidemic Control Center implemented a range of strategies to protect LTCF residents. These included early containment measures to allow time for preparing pharmaceutical intervention, the establishment of infection prevention and control guidelines, the implementation of comprehensive screening and testing protocols, the prioritization of vaccination for both residents and staff, and the expansion of the national stockpile of oral antiviral agents.
