AI Article Synopsis

  • The study evaluated the safety and practicality of two techniques, the double-loop technique (DLT) and the DeBakey clamp, used to address pulmonary artery (PA) issues during lung surgery from April 2007 to August 2022.
  • Researchers reviewed patient records and measured changes in PA diameter before and after surgery, finding no significant injuries or complications related to either clamping method.
  • Results indicated that both techniques had minimal impact on PA diameter, with DLT proving to be a practical option for managing PA bleeding after achieving primary hemostasis.

Article Abstract

Objectives: We analysed our clinical experience using silk sutures [the double-loop technique (DLT)] or DeBakey type vascular clamp (DeBakey clamp) for pulmonary artery (PA) troubles during anatomical lung resection to validate its practicality and safety.

Methods: We retrospectively reviewed the records of patients who underwent either of the above clamping techniques during anatomical lung resection at our hospital between April 2007 and August 2022. We measured the PA diameter at the occlusion site on computed tomography images acquired within 1 year pre- and postoperatively. The difference between pre- and postoperative diameters of the occlusion sites was calculated as the change in the PA diameter. We zoned the occlusion site of the PA to adjust for variation. PA deformation was evaluated as an adverse event caused by clamping.

Results: Ultimately, 27 and 26 patients who underwent the DLT and DeBakey clamp, respectively, were included. No additional injury due to the clamp procedure was found in either group. For zone R1/L1, defined as the main PA, the median changes in the PA diameter were 0.02 (-0.7 to 0.27) mm for the DLT and 0.36 (-0.28 to 0.89) mm for the DeBakey clamp. No significant differences were observed between the 2 groups (P = 0.106). Furthermore, no aneurysms, dissections, or stenoses were found in either group.

Conclusions: The DLT and DeBakey clamp had only minimal effects on the occlusion site of the PA. The DLT is a practical thoracoscopic technique for PA bleeding when primary haemostasis has been achieved.

Bypass Grafting for Myocardial Infarction From Endocarditis Embolus to the Left Anterior Descending.

Ann Thorac Surg Short Rep

December 2024

Department of Thoracic and Cardiovascular Surgery, Cleveland Clinic, Cleveland, Ohio.

A 67-year-old man with aortic valve endocarditis presented with acute ST-segment elevation myocardial infarction and was found to have embolic vegetation occlusion of the left anterior descending coronary artery. This patient was successfully treated with early aortic valve replacement, extraction of a vegetation embolus, and coronary artery bypass grafting over the site of extraction.

Background: Variability in long-term endovascular treatment outcomes for intracranial aneurysms has prompted questions regarding the effects of these treatments on aneurysm hemodynamics. Endovascular techniques disrupt aneurysmal blood flow and shear, but their influence on intra-aneurysmal pressure remains unclear. A better understanding of aneurysm pressure effects may aid in predicting outcomes and guiding treatment decisions.

Background: Hepatic artery infusion pump (HAIP) chemotherapy is a locoregional treatment for intrahepatic malignancies. HAIPs are surgically implanted, and the catheter tip is typically inserted into a ligated gastroduodenal artery stump. Potential complications at the catheter insertion site include dehiscence, pseudoaneurysm or extravasation, and adjacent hepatic arterial stenosis and thrombosis.

Neutrophils are peripheral blood-circulating leukocytes that play a pivotal role in host defense against bacterial pathogens which upon activation, they release web-like chromatin structures called neutrophil extracellular traps (NETs). Here, we analyzed and compared the importance of myeloid differentiation factor 88 (MYD88), peptidyl arginine deiminase 4 (PAD4), and gasdermin D (GSDMD) for NET formation in vivo following sepsis and neutrophilia challenge. Injection of lipopolysaccharide (LPS)/E.
