AI Article Synopsis

  • - The infratemporal fossa (ITF) poses surgical challenges due to its deep location and complex vascular structures, but a combined surgical approach can allow for effective tumor removal.
  • - A 69-year-old male patient presented with symptoms related to an ITF tumor, which was successfully excised through a combination of surgical techniques without causing damage to surrounding tissues.
  • - The case highlights the advantages of this method, such as improved visibility, sufficient working space for tumor removal, and minimal impact on recovery due to the use of smaller incisions.

Article Abstract

Background: The deep location of infratemporal fossa (ITF) combined with the abundant vascular plexus in it increased the difficulty of removing the mass in ITF through endoscope surgery approach. However, under appropriate circumstances, the excision of ITF tumors through a combined prelacrimal recess, Caldwell-Luc, and distal intraoral approach can be safely performed with minimal impact on the surrounding tissues.

Case Presentation: The Department of Neurology received a 69-year-old male patient who had been experiencing headache, dizziness, and numbness from the mastoid region of his left ear to the corner of his mouth for a duration of 22 days. Cranial magnetic resonance imaging revealed the presence of a tumor located in the ITF. Following transfer to our department, surgical intervention was performed using a combined approach involving the prelacrimal recess, the anterior wall of maxillary sinus, and lateral ITF to successfully remove the tumor. Postoperative pathologic examination confirmed schwannoma as its nature. The patient was discharged in excellent condition without any functional impairment.

Conclusions: On the basis of this case, the authors believe that this combined approach can offer a distinct endoscopic perspective and adequate surgical workspace, which is crucial for tumor removal while preserving the integrity of surrounding normal tissues. Moreover, the utilization of multiple small incisions has minimal impact on postoperative recovery.

Purpose: The aim of this study is to describe the maximum exposure of the infraorbital region via the orbital floor using the transnasal prelacrimal recess approach (PLRA), and to provide an anatomical basis for treating lesions in the infraorbital region.

Methods: Ten freshly injected frozen heads were dissected using the PLRA. The orbital floor was removed along the border of the medial infraorbital quadrangle, and the periorbita was opened to expose the infraorbital region.

Prelacrimal-Transmaxillary Approach to Lateral Sphenoid Recess Skull Base Defects.

The lateral recess of a well-pneumatized sphenoid sinus is challenging to access surgically. Traditional methods require the use of multiple angled endoscopes and curved instruments which may limit visualization. We describe a prelacrimal-transpterygoid/maxillary approach which offers direct access to this region with a 0° endoscope.

Maxillary sinus pathology is commonly approached through endoscopic middle meatal antrostomy (MMA). However, MMA does not provide full exposure to certain aspects of the maxillary sinus, such as alveolar, zygomatic, and prelacrimal recesses. In these hidden maxillary sinus niches, a combination approach with MMA is required for better exposure and access.

Prelacrimal recess approach for maxillary sinus inverted papilloma: a 15-year experience from a single center.

Eur Arch Otorhinolaryngol

September 2024

Department of Otolaryngology Head and Neck Surgery, Beijing Tongren Hospital, Capital Medical University, Beijing, China.

Purpose: This large retrospective, single-center, follow-up study investigated the endoscopic prelacrimal recess approach (PLRA) for treating maxillary sinus inverted papilloma (MSIP).

Methods: Between January 2007 and November 2022, patients with MSIP treated with PLRA were enrolled. Data on clinical manifestations, imaging, and surgical procedures were collected.
