Incessant focal atrial tachycardia may be encountered in the pediatric age group although it is rarely seen. Ablation using radiofrequency or cryothermal energy is the preferred method for drug-resistant cases. Recently, 3D electroanatomic mapping systems have been increasingly used for mapping and ablation.

We report on the characteristics and the ablation procedure of a focal atrial tachycardia originating from the non-coronary aortic cusp. The electrophysiological features of the tachycardia included: (i) incessant pattern; (ii) easy induction and termination by atrial stimulation; (iii) earliest right atrial activation at the para-hisian area; and (iv) termination by adenosine. Left-sided mapping revealed the earliest atrial activation during the tachycardia at the non-coronary aortic cusp.

The congenital form of His bundle tachycardia is an uncommon pediatric arrhythmia. We report the case of a 7-year-old child with tachycardiomyopathy. The incessant arrhythmia, detected in infancy, was resistant to amiodarone and beta-blockers.

Introduction: The effects of adenosine on atrial tachycardia (AT) remain controversial, and the mechanistic implications of adenosine termination have not been fully established. The purpose of this study was to elucidate the differential effects of adenosine on focal and macroreentrant AT and describe the characteristics of adenosine-sensitive AT.
Methods And Results: Thirty patients received adenosine during AT.
