Cutaneous squamous cell carcinoma (CSCC) overlying an arteriovenous fistula (AVF) is rare and presents unique challenges. This case report describes a method of fistula preservation after CSCC excision using a flow-through venous free flap. The saphenous vein of the venous flap was used as flow-through segment for AVF preservation. The flap was inserted along the dorsal aspect of the forearm wound and microvascular anastomosis of the arterial inflow was completed using a vein just proximal to the radiocephalic fistula anastomosis. Venous outflow was established by creating an end-to-end vascular anastomosis between the cephalic vein and the greater saphenous vein. A separate subcutaneous vein was used to provide a low-pressure outflow for the flap to avoid congestion. This case demonstrates an option for AVF preservation that has not been previously described. It also highlights the importance of a multidisciplinary approach for the safe treatment of CSCCs overlying AVFs.

Amyloid related imaging abnormalities (ARIA) are side effects of anti-Abeta immunotherapy, which are most frequent and associated with greater morbidity in ApoE4 individuals. ARIA are characterized by neurovascular inflammation, leading either to increased vascular permeability and edema (ARIA-E), or to more severe vascular damage and microhemorrhages (ARIA-H). The mechanisms by which Abeta immunotherapy leads to ARIA remain to be established but may involve overload of the cerebral microvasculature by Abeta released from amyloid plaques.

Introduction Defects of the lower extremity often require free tissue transfer to provide adequate soft tissue reconstruction. Patients typically undergo a postoperative dangle protocol to condition the flap to withstand the increase in venous pressure. The purpose of this study was to evaluate the safety and postoperative length of stay after early initiation of dangle.

Objectives: To assess the use of occipital vessels for microvascular anastomosis in head and neck free tissue transfer reconstruction.
Methods: A literature search was undertaken to identify studies utilizing the occipital vessels for microvascular anastomosis in free tissue transfer. Following literature review, 30 anatomic cadaveric dissections on 15 fresh unfixed cadavers were performed to evaluate the occipital artery and identify a reliable vein within reasonable proximity.

There is limited available data regarding the impact of cycle length (CL) prolongation when converting atrial fibrillation (AF) to organized atrial tachycardia (AT) and its effect on clinical outcomes. We retrospectively screened and included a cohort of 132 patients with persistent or long-standing persistent AF who underwent circumferential pulmonary vein isolation (CPVI) and left atrial substrate modification (LASM) between January 2015 and October 2019. In all 132 consecutive patients, persistent AF was successfully converted into organized AT.

Background: In cirrhotic patients, portal hypertension increases mortality after surgery. We evaluated the impact of pre-operative transjugular intrahepatic portosystemic shunt (TIPS) on the outcomes of bariatric surgery in cirrhosis.
Methods: Multicentric retrospective cohort.
