  • Stereotactic body radiation therapy (SBRT) for ultra-central lung tumors has high toxicity rates.
  • The proximal bronchial tree (PBT) was divided into seven sections to study differences in radiosensitivity.
  • A risk-adapted SBRT regimen with a specific dose has shown excellent tumor control and lower severe toxicity rates, with a modeling study suggesting a dose threshold for low bronchial toxicity.

Article Abstract

•Stereotactic body radiation therapy (SBRT) for ultra-central lung tumors is associated with high toxicity rates.•To evaluate differences in radiosensitivity within the proximal bronchial tree (PBT), the PBT was sub-segmented into seven anatomical sections.•A risk-adapted SBRT regimen of EQD2_10 = 54.4 Gy in 8 or 10 fractions results in excellent local control and low rates of severe toxicity.•Data from a recent -analysis, the NORDIC Hilus trial and dosimetric data from this study were combined to create a NTCP model.•A dose threshold of EQD2_3 = 100 Gy to the PBT or any of its subsegments is expected to result in low rates of severe bronchial toxicity.

Optimization of a micro-scale air-liquid-interface model of human proximal airway epithelium for moderate throughput drug screening for SARS-CoV-2.

Background: Many respiratory viruses attack the airway epithelium and cause a wide spectrum of diseases for which we have limited therapies. To date, a few primary human stem cell-based models of the proximal airway have been reported for drug discovery but scaling them up to a higher throughput platform remains a significant challenge. As a result, most of the drug screening assays for respiratory viruses are performed on commercial cell line-based 2D cultures that provide limited translational ability.

SARS-Cov-2 is a corona virus that causes COVID-19 disease, a viral infection responsible for the pandemic decreed by the World Health Organization in March 2020. Angiotensin-converting enzyme 2 (ACE-2) functions as the main receptor for SARS-Cov-2. The study aimed to detect the expression of ACE-2 in the gastrointestinal tract, kidney, and lung in the rhesus monkeys and squirrel monkeys.

A man in his 60s, who had undergone surgery for rectal cancer, liver metastases, and lung metastasis, had a past history of myocardial infarction and ventricular fibrillation with reduced cardiac functions. He was referred to our department because of a pulmonary nodule shadow in the S2 right upper lobe and a bronchial cast shadow along the B2 bronchus. Robot-assisted thoracoscopic right S2 segmentectomy was performed and intraoperative bronchoscopy revealed a polyp-like tumor within B2a.

Article Synopsis
  • Hemoptysis, the coughing up of blood, can stem from various serious underlying conditions, but often involves high-pressure bronchial arteries; Bronchial artery embolization (BAE) is typically the first-line treatment despite varying long-term success rates.
  • A 12-year-old boy with a history of massive hemoptysis underwent multiple BAEs and recent imaging confirmed the location of previous coils; however, he experienced another episode where traditional intervention was complicated by prior treatments.
  • The medical team opted for a thoracoscopic approach, successfully isolating and ligating the problematic vessel, leading to the patient's stabilization and discharge after four days.
