[Pathocharacterologic reactions in adolescence].

Zh Nevropatol Psikhiatr Im S S Korsakova

Published: March 1987

Pathocharacterological reactions have been studied in a long-term follow-up (9-12 years) of 58 youths aged 14-18 years. These reactions were observed more frequently (P less than 0.05) in teenagers with a premorbid accentuation of the character of the epileptoid, unstable, hyperthymic, or hysteroid types. The studied reactions are classified in accordance with the characterological principle. Recommendations on rehabilitation of the above group of patients are presented. The questions of the differential diagnosis are considered. On the whole a stable social adaptation was observed in 72.4% of patients despite a considerable duration of pathocharacterological reactions in some of them.

An analysis of the interaction between pathocharacterological and psychopathological (affective, anxious-phobic and hysterical) phenomena within "borderline psychiatry" (reactions, phases, development) gives evidence to the existence of 2 clinically heterogeneous variations of comorbid interactions. The first variation: primary psychopathological syndromes manifesting without any significant correlation with personality disorders and transform into pathocharacterological ones (neurotic, postreactive) personality development by means of "amalgamating" mechanism. The second variation: secondary (in regard to personality disorders) psychopathological phenomena forming on the basis of constitutional personality traits by means of "splitting off" mechanism and are defined as personality disorders with predisposition to manifest positive psychopathological symptomatology.

[Typology of psychogenic mania].

Zh Nevropatol Psikhiatr Im S S Korsakova

August 1991

In order to typologically differentiate between psychogenic manias, 20 patients (18 women and 2 men) aged 23 to 70 years with manic conditions that developed because of mental injury were examined. Based on the differences in certain clinical parameters pertaining mainly to the time-course of changes in the manic syndrome, 2 types of psychogenic mania+ were distinguished: genuine manic reactions and psychosis-provoked manic phases. Correlations were established between the type of psychogenic mania and the character of constitutional predisposition.

Based on the clinical and catamnestic examination (with the time of the catamnesis being from 5 to 10 years) of 64 persons discharged from the army with a diagnosis of psychopathy, a study was made of certain characteristics of the formation of juvenile mental disorders and further time-course of changes in the illness. The clinical manifestations that contributed to the discharge from the army appeared to vary. In part of the examined, the status could be regarded as decompensation with steady pathocharacterological reactions and neurotic disorders (in accentuated personalities).

Children with behavioural disorders presented changes in manifestations of different components of the orientation response and impairment of their speed and order of reduction.
