The modified pedicled internal mammary osteomyocutaneous chimeric flap is a powerful option for head and neck reconstruction in patients not suitable for free tissue transfer. In this article, the senior author's (K.C.) technique for flap elevation is described in the context of a patient with mandibular osteoradionecrosis resulting in severe crossbite and trismus after multiple failed attempts at reconstruction with free tissue transfer. The modified pedicled internal mammary osteomyocutaneous chimeric flap was chosen as it offered intraoral lining, extraoral soft tissue, and vascularized bone for mandibular reconstruction without requiring free tissue transfer. The flap dissection as well as the risks, benefits, and indications for this flap are described herein. The modified pedicled internal mammary osteomyocutaneous chimeric flap is a technically complex reconstructive option reserved for situations in which conventional methods have been exhausted. It offers an eloquent solution for patients who otherwise may have no options.

V-to-Y advancement flap is a successful repair technique that preserves vascular and tissue integrity adopted after Mohs micrographic surgery to repair cutaneous defects on the head and neck. However, defects at the lateral distal nasal ala requires large extension beyond cosmetic margins that increase risk of skin webbing, an undesired result on a cosmetically sensitive location to the patient. In this article, we present a novel approach to modifying the procedure employing the V-to-Y advancement flap by truncated the trailing end of the island pedicle to allow for successful healing and better patient satisfaction.

Reduction of very huge breast-gigantic macromastia-is a challenge to breast surgeons in choosing the right procedure to obtain an optimal outcome. The feasibility of a superomedial pedicle (SMP) with some modifications proves to be a good option to achieve a viable nipple areolar complex (NAC) with good size and shape after good resection above 800 g. Out of the 35 patients with 70 breast reductions, 15 can be considered gigantic macromastia with reductions above 800 g.

Background: Transgenic plants expressing proteins that target the eggs of the ubiquitous plant pest Bemisia tabaci (whitefly) could be an effective insecticide strategy. Two approaches for protein delivery are assessed using the mCherry reporter gene in transgenic tomato plants, while accommodating autofluorescence in both the plant, phloem-feeding whitefly and pedicle-attached eggs.
Results: Both transgenic strategies were segregated to homozygous genotype using digital PCR.
