Background: The administration of propofol and methylene blue (MB) can be associated with the appearance of prolonged green urine discoloration, particularly in patients with heart failure (HF) concomitant with renal and liver dysfunction. Understanding the reasons behind this phenomenon is of clinical significance.
Case Summary: A 79-year-old woman with a history of HF experienced dyspnoea and persistent green urine discoloration for a week, leading to her hospitalization for acutely decompensated HF. A recent dual-chamber rate-modulated-pacemaker implantation had necessitated propofol sedation and the administration of 100 mg of MB due to methaemoglobinaemia. Upon admission, the patient exhibited elevated levels of brain natriuretic peptide (BNP) and liver function tests, as well as a significant decrease in glomerular filtration rate (GFR). Initial therapy with intravenous furosemide yielded an inadequate response, requiring the initiation of combined diuretic therapy (CDT). The patient's condition improved with CDT, resulting in the normalization of BNP, liver function tests, and GFR, along with the restoration of normal urine colour lasting 12 days.
Discussion: Our case report sheds light on the complex interaction between drug metabolic pathways and their potential for prolonged side effects, particularly in patients with multiorgan dysfunction. The association between propofol, MB, and green urine discoloration in the context of HF warrants further investigation, emphasizing the need for increased awareness of drug interactions and their implications in complex clinical scenarios.

In the present study, a norfloxacin (NFX) fluorescent probe was tailored for the spectrofluorometric measurement of cefepime (CFP). The proposed approach measured the quenching effect of CFP on the fluorescence intensity of NFX in acetate buffer solution. The obtained results show that CFP strongly quenches the fluorescence of NFX in a static mechanism.

Background: The treatment of transplant ureteral stricture (TUS) has been a great challenge, and there is limited experience with indocyanine green (ICG) fluorescence-guided robotic Boari flap-pelvis anastomosis to identify ureteral stenosis segments (especially long-segment) and their postoperative blood supply. We report case series of ureteral strictures treated with ICG fluorescence-guided robotic Boari flap-pelvis anastomosis in our center.
Case Description: We retrospectively collected clinical data of six patients diagnosed with long-segment even full-length TUS who underwent robotic Boari flap-pelvis anastomosis with the assistance of modified distribution of robotic ports and ICG fluorescence between June 2022 and June 2024, focusing on postoperative renal function, stenosis recurrence, and urinary fistulae.

Herein, a novel spectrofluorometric sensor is proposed for the sensitive analysis of two nonfluorescent mucolytic drugs, namely, acetylcysteine (ACT) and carbocisteine (CST), utilizing the newly synthesized 2-[(2-hydroxyethyl)-(2,8,10-trimethylpyrido[2',3':3,4]pyrazolo[1,5-a]pyrimidin-4-yl)-amino]-ethanol as a fluorescence probe (Flu. Probe). This fluorophore exhibits fluorescence emission at 445 nm upon excitation at 275 nm.
