Bempedoic acid: new evidence and recommendations on use.

Curr Opin Lipidol

Department of Cardiology and Congenital Diseases of Adults, Polish Mother's Memorial Hospital Research Institute.

Published: February 2024

Purposes Of Review: Cardiovascular diseases (CVDs) are the leading cause of mortality and morbidity worldwide. Recognizing the importance of dyslipidemia treatment in the prevention of cardiovascular events has become a part of standard clinical practice. Desired values of LDL cholesterol (LDL-C) have become lower and lower in the last few decades, as evidenced by the most recent guidelines. Therefore, efforts to lower LDL cholesterol concentrations with conventional therapies and combinations of lipid-lowering therapy may not be successful in a high proportion of patients.

Recent Findings: Bempedoic acid is a novel agent, first in-class ATP Citrate Lyase (ACL) inhibitor, which targets biosynthesis of the cholesterol in the liver. Considering the results of phase 3 studies, it has been approved for sole use for dyslipidemia treatment for patients who are statin-intolerant or in combination with statin-ezetimibe for those suffering from familial hypercholesterolemia or ASCVD and unable to reach targeted LDL-C values.

Summary: Bempedoic acid has proven beneficial for further reduction of LDL cholesterol for targeted groups of patients. It is not only efficient but also a well tolerated, affordable, and available agent whose place in lipid-lowering management is yet to be fully understood with new data collected from ongoing clinical research. In this review we suggest the place of bempedoic acid in lipid-lowering management.

Oral bempedoic acid (NEXLETOL in the USA; Nilemdo in the EU) and the fixed dose combination (FDC) of bempedoic acid/ezetimibe (NEXLIZET in the USA; Nustendi in the EU) are approved to reduce cardiovascular (CV) risk in statin-intolerant patients who are at high risk for, or have, CV disease. A first-in-class therapy, bempedoic acid inhibits the adenosine triphosphate-citrate lyase enzyme in the cholesterol biosynthesis pathway. In the multinational phase III CLEAR Outcomes trial in statin-intolerant patients, once-daily bempedoic acid 180 mg significantly reduced the risk of the primary endpoint (a four-component major adverse CV event composite of CV death, nonfatal myocardial infarction, nonfatal stroke, or coronary revascularization) compared with placebo.

The year 2024 has witnessed substantial advancements in interventional cardiology, encompassing both coronary and structural interventions. In coronary field, trials have explored percutaneous innovations for coronary lesions, strategies for managing post‑infarction cardiogenic shock and non‑invasive approaches for guiding revascularization. The uploaded guidelines for chronic coronary syndromes emphasize individualized care, integrating modalities such as fractional flow reserve (FFR), intravascular ultrasound (IVUS), optical coherence tomography (OCT) and new teatments, including cochicine, GLP-1 receptor agonists and bempedoic acid.

Current Perspectives of Diabetic Dyslipidemia and Treatment Modalities.

Curr Med Chem

January 2025

Cukurova University, Faculty of Medicine, Division of Endocrinology, Adana, Turkey.

Introduction: Diabetes mellitus is associated with an increased risk of atherosclerosis related to dyslipidemia. Although the terms hyperlipidemia and Diabetes Mellitus [DM] or diabetic dyslipidemia are interrelated to each other, these two conditions have some differences.

Aim: This study aimed to highlight possible mechanisms of hyperlipidemia and/or dyslipidemia in diabetic patients, which can be treated with available and newer hypolipidemic drugs.

Cutting-edge lipid-lowering pharmacological therapies: Improving lipid control beyond statins.

Hipertens Riesgo Vasc

Hospital Pharmacist Manager, Pharmaceutical Department, Asl Napoli 3 Sud., Italy. Electronic address:

Statins are crucial for both the prevention and management of atherosclerotic cardiovascular disease (ASCVD). However, even with optimized statin therapy, a significant residual risk of ASCVD remains, highlighting the need for innovative approaches to lipid-lowering therapies (LLT) that more effectively target low-density lipoprotein cholesterol (LDL-C) and other atherogenic lipoproteins. Recently, novel pharmacologic agents have been introduced for the management of dyslipidemia.

Article Synopsis
  • Atherosclerotic cardiovascular disease (ASCVD) is a major global health issue, with high levels of low-density lipoprotein cholesterol (LDL-C) linked to increased cardiovascular risk.
  • Numerous studies confirm that lipid-lowering therapies (LLT) are effective; however, there is a gap between guidelines and actual clinical practice regarding LDL-C targets.
  • The review suggests new strategies beyond traditional LDL-C management, such as combination therapies and emerging drugs, while also introducing the potential use of polygenic risk scores for personalized treatment approaches in ASCVD prevention.
