Social Determinants of Health and Disparities in Thyroid Care.

Published: February 2024

Background: There have been documented racial and ethnic disparities in the care and clinical outcomes of patients with thyroid disease.

Context: Key to improving disparities in thyroid care is understanding the context for racial and ethnic disparities, which includes acknowledging and addressing social determinants of health. Thyroid disease diagnosis, treatment, and survivorship care are impacted by patient- and system-level factors, including socioeconomic status and economic stability, language, education, health literacy, and health care systems and health policy. The relationship between these factors and downstream clinical outcomes is intricate and complex, underscoring the need for a multifaceted approach to mitigate these disparities.

Conclusion: Understanding the factors that contribute to disparities in thyroid disease is critically important. There is a need for future targeted and multilevel interventions to address these disparities, while considering societal, health care, clinician, and patient perspectives.

Universal salt iodisation (USI) plays an essential role in the provision of iodine (I) to populations worldwide. Countries adopting USI programmes, adhering to strict criteria laid down by expert organisations such as the Iodine Global Network, are estimated to have reduced the prevalence of I deficiency by 75% (protecting 720 million individuals worldwide). Despite this success, doubts have been raised as to the desirability of continuing such programmes because of (a) the need to reduce salt intake for cardiovascular prevention and (b) the induction of thyroid autoimmunity.

Objective: Successful embryo implantation is contingent upon the intricate interaction between the endometrium and the blastocyst. Recurrent implantation failure (RIF) signifies the clinical challenge of failing pregnancy post-transfer of high-quality embryos, fresh or frozen, in at least three in vitro fertilization (IVF) cycles, often in women under 40 years. Recent studies identify impaired blastocyst maternal tissue communication among recurrent implantation failure causes.

Article Synopsis
  • This research investigates the potential biomarkers and metabolic changes in cerebrospinal fluid (CSF) of patients with autoimmune encephalitis (AE) compared to healthy controls.
  • A total of 21 potential biomarkers were identified, with elevated levels of pyruvic and oxoglutaric acids, pointing to a dysregulated TCA cycle in AE patients.
  • Additionally, the study highlights differences in unsaturated fatty acid metabolism, indicating that these metabolic pathways may play a significant role in understanding the pathology of AE.

Objective: This systematic review seeks to evaluate the prevalence of local symptoms in patients with benign thyroid disease as described in the literature.

Data Sources: A literature search was conducted across PubMed, Embase, Medline, and Cochrane databases.

Review Methods: Crude symptom prevalence was obtained by addition of data across studies that reported local symptoms, and adjusted symptom frequency was calculated using a random effects model.

Background: Although thyroid cancer is associated with low mortality rates, significant racial disparities in thyroid cancer outcomes have not been adequately studied in Asia. Moreover, the Asian population consists of different ethnic groups that are not homogeneous. This study aimed to perform a population-based analysis of survival outcomes and prognostic factors in thyroid cancer patients.
