Background: Aneurysms of the lower basilar artery (BA) are rare, accounting for <1% of all intracranial aneurysms. This location has been described as "No man's land" since it poses a potential challenge for microsurgery. Recently, endovascular treatment has become an alternative option; however, there are some disadvantages regarding the obliteration rate, patency of the parent, and perforating arteries.

This case report describes a patient who presented with devastating stroke-like symptoms secondary to a cystic brain lesion that was confirmed to be glioblastoma without significant symptomatology before her dramatic presentation. It further highlights the aggressive nature and the swift growth of the tumor in a short period of time. A 55-year-old female with no significant past medical history presented to the emergency department in a comatose state.

Background: The dorsal midbrain, an anatomically intricate region, presents significant challenges for traditional surgical interventions due to the heightened risk of vascular and neurological injury, and the necessity of brain tissue retraction.
Methods: This study retrospectively reviewed the case of a 29-year-old male diagnosed with a cavernous malformation located in the dorsal aspect of the left midbrain. The patient underwent resection via the endoscopic occipital transtentorial approach (EOTA) in July 2024.
