The endovascular approach is widely used in the management of aortic isthmic rupture. Even if it remains less invasive than conventional surgery, a life-threatening complications are possible. We report the case of a young female patient presenting a stent-graft migration during the deployment with total obstruction of the supra-aortic vessels. We describe the therapeutic management with a cerebral rescue procedure followed by a delayed surgical repair.

The Long-Term Progression of Aneurysmal Disease in Common Iliac Arteries After Standard EVAR and Its Clinical Implications.

Int J Vasc Med

November 2024

Second Department of Surgery, Division of Vascular Surgery, "G. Gennimatas" General Hospital of Thessaloniki, School of Health Sciences, Faculty of Medicine, Aristotle University of Thessaloniki, Thessaloniki, Greece.

Article Synopsis
  • This study examines the long-term effects of elective Endovascular Aneurysm Repair (EVAR) on the common iliac arteries (CIAs) using various modern aortic endografts in a cohort of 168 patients.* -
  • Over a 5-year follow-up, significant increases in the diameter (11.7% at 24 months, 22.8% at 60 months) and length of CIAs were observed, indicating a concerning progression of aneurysmal disease.* -
  • The research found that increased CIA dilatation significantly predicted long-term EVAR failures, highlighting the need for comprehensive follow-up to ensure ongoing effectiveness of the procedure.*

Background: Stent graft-induced new entry (SINE) after thoracic endovascular aortic repair (TEVAR) is a serious adverse event which leads to stent graft migration and rupture. SINE is known to be more frequent in cases of chronic dissection and oversizing. However, few studies have evaluated the influence of hemodynamic stress on SINE in patients with chronic dissection.

Objectives: The frozen elephant trunk (FET) technique is effective for treating extended aortic arch aneurysms. This study compares hand-made and factory-made devices in this context.

Methods: A retrospective case-control study was conducted on 68 patients who underwent FET for distal aortic arch aneurysm at our institution.

Introduction: The Anaconda aortic stent graft is a trimodular endovascular stent graft with an active infrarenal fixation suitable for the treatment of infrarenal abdominal aortic aneurysms with an infrarenal neck angulation ≤90°. A unique magnet-based mechanism facilitates the cannulation of the contralateral leg.

Areas Covered: The present article provides a complete description of the third-generation Anaconda endograft, the Anaconda One-Lok, its clinical performance and the related technical and mechanical characteristics as well as a brief comparison between itself and other similar endografts.

A New Bailout Maneuver to Manage Type IIIa Endoleak Due to Displaced Renal Bridging Stent Graft in Narrow Aorta.

J Endovasc Ther

October 2024

Department of Vascular Surgery, School of Medicine, University Hospital and Trust of Verona, University of Verona, Verona, Italy.

Purpose: To present endovascular management of an intraoperative type IIIc endoleak (EL) in a patient with migration of the right renal artery (RRA) bridging stent graft (BSG) during branched aortic aneurysm repair.

Technique: The technique is demonstrated in an 80-year-old woman who underwent branched endograft repair of a symptomatic 6-cm type II TAAA. The t-Branch thoracoabdominal stent graft was positioned without difficulty.
