In Video S1, a new surgical technique for hybrid primary tracheoesophageal (TE) puncture in stapler-assisted total laryngectomy is shown. The video describes the surgical steps of the procedure and illustrates some tips and tricks. The procedure incorporates an upper mini-pharyngotomy to enable retrograde placement of the voice prosthesis (VP), eliminating the need for rigid esophagoscopy. This has made it possible to exploit, without additional risks, the potential of the stapler combined with primary TE puncture and VP placement. In our experience, this hybrid procedure in stapler-assisted total laryngectomy is not related to adverse events such as pharyngocutaneous fistula (PCF), hypertonicity, and functional complications. Therefore, it can be considered a valid technique that allows for easy insertion of a primary voice prosthesis also in case of mechanical sutures.

Article Synopsis
  • The study reviews literature on stapler-assisted closed total laryngectomy (TL) and compares it to traditional manual methods.* -
  • Research shows that stapler-assisted TL results in fewer pharyngocutaneous fistulas (PCF), shorter surgery times, reduced hospital stays, and quicker resumption of oral feeding.* -
  • A case series of five patients demonstrated successful outcomes with stapler-assisted TL, including innovative reconstruction techniques, suggesting it is a safe option for certain patients with laryngeal tumors.*

Introduction: Stapling-assisted closure of pharyngeal mucosa during total laryngectomy was described in 1973.Few authors have demonstrated that this technique provides a faster and more reliable pharyngeal closure with ashorter operative period. However, the simultaneous placement of tracheoesophageal prostheses is quitechallenging and affects the patient's speech rehabilitation.

With the advancements in the medical field, many innovations in medical devices have happened. Using a surgical stapler to close the laryngectomy defect without opening the pharynx is particularly advantageous in a total laryngectomy (TL). However, performing the tracheoesophageal puncture (TEP) during stapler closure of the larynx has not been widely advocated, due to the fear of complications related to the procedure.

Background: Esophageal cancer (EC) is one of the most common malignant tumor types. Surgery is considered the treatment of choice for patients with early- and mid-stage EC. However, because of the traumatic nature of EC surgery and the need for gastrointestinal reconstruction, high rates of postoperative complications such as anastomotic leakage or stenosis, esophageal reflux, and pulmonary infection exist.
