Clinico-roentgenologic and bacteriologic efficacy and safety of rifapex (rifapentin) were investigated in the complex therapy of 90 patients with newly recorded drug-susceptible tuberculosis. The author focuses on the pathoanatomical and pathophysiological aspects of chronic pleuritis, ways of formation of chronic pulmonary heart. The clinico-roentgenologic classification proposed by the author suggests limits to the conservative treatment of pleuritis together with the indications for surgical intervention preventing the above complication. Using the criteria of the author's classification, there have been carried out a total of 211 surgical interventions; of these, 203 were pleurectomies.

The results of the clinico-roentgenologic and functional examination of 434 patients with different forms of pulmonary tuberculosis were subjected to analysis. The obstructive syndrome of ventilation disorders was detected in more than 60% of pulmonary tuberculosis patients. The prevalent significance in its pathogenesis was played by disorders of patency of fine bronchi and air distribution in the lungs.

On the basis of observation of 65 patients with focal traumatic injury to the brain (hematoma of mater encephali was noted in 24, intracerebral--in 27, a focus of type III brain contusion--in 24, type IV--in 11, multiple hematomas and foci on contusion-crush--in 21), the criteria for their non-surgical treatment have been substantiated. Choice of a method for the treatment depends on the sizes of a hematoma, focus of contusion-crush, location of an injury, degree of its influence on the brain structures (deformation, shift), value of intracerebral pressure, dynamics of the clinico-roentgenologic symptoms.
