Rationale: The manuscript aimed to show that an unmeasurable capillary C-reactive protein (CRP) should be a red flag that can indicate a possible severe hematological pathology.

Patients Concerns And Diagnoses: The authors present 3 case reports of children with fever examined at the pediatric emergency department. Fever is among the most frequently exhibited symptoms of acute pediatric infectious diseases. However, sometimes fever can be the manifestation of other serious noninfectious diseases. CRP is a marker widely used in clinical pediatric practice to help us evaluate inflammation and possible bacterial infection. All mentioned patients had unmeasurable CRP from capillary blood, even though venous CRP ranged from 14 to 21 mg/L. All of the patients were consequently diagnosed with severe hemato-oncological disease. Possible explanations are that a change in blood viscosity or an elevation of circulating immune complexes in the blood of patients with leukemia leads to malfunctioning immunoturbidimetry measurement.

Lesson: Although these findings are very interesting and could lead to faster recognition of acute leukemia in pediatric clinical practice, further prospective study is needed for their confirmation.

Article Synopsis
  • - The direct Fick principle is used to calculate cardiac output (CO) to identify conditions like exercise pulmonary hypertension (ePH) by comparing arterial and mixed venous values, which can lead to inconsistencies based on the presence of an arterial catheter.
  • - A study analyzed 296 invasive cardiopulmonary exercise tests, revealing significant discrepancies in oxygen saturation and CO calculations between arterial and non-arterial methods, which resulted in a higher rate of data loss and misclassification of pre- and post-capillary ePH.
  • - The findings suggest that relying on non-arterial measures during exercise testing can misclassify ePH, particularly affecting patients with conditions like scleroderma, Raynaud's, and black individuals

Background: Normal hemoglobin is a tetrameric structure, consisting of two alpha-globin chains and two nonalpha (beta, gamma, delta) chains. Hemoglobinopathies occur when the presence of gene mutations affect the molecular structure or expression of the globin chains.

Methods: We reported the case of a 9-year-old Chinese girl who presented with abnormal low oxygen saturation values on pulse oximetry and no oximetry results were obtained during blood gas analysis (BGA).

Objectives: Hyperthyroidism is a common endocrinopathy affecting middle-aged to elderly cats, with multisystemic repercussions. Hyperthyroid humans show decreased lung compliance and increased cardiac output with subsequent left heart failure leading to pulmonary capillary congestion. Prognosis worsens with the development of increased pulmonary vascular pressures (ie, pulmonary hypertension [PH]) in hyperthyroid humans.

Aim: To study the macular structure and vasculature in consecutive nanophthalmic eyes using optical coherence tomography angiography.

Methods: This is a prospective, multicentre, cross-sectional study of patients with nanophthalmos (one or both eyes). The superficial and deep foveal avascular zones (FAZ) were measured both manually and with the machine's built-in automated measurement tool.
