Idiopathic Chylothorax During Pregnancy: A Case Report.

The diagnosis of chylothorax during pregnancy is rare and is generally triggered during labor. The study of pleural fluid and imaging studies can facilitate diagnosis, even though determining its etiology can be complicated. This case involves a young pregnant woman presenting with disabling dyspnea and chest pain due to pleural effusion. The conventional study of the pleural fluid was not conclusive; however, the measurement of chylomicrons guided us to determine the diagnosis of chylothorax. Based on imaging studies and specific markers, some of the main etiologies were not determined, leading to classification as idiopathic. After drainage and dietary adjustment, a significant improvement in symptoms was achieved with the help of an interdisciplinary team, which is crucial for the prevention of complications and adequate resolution of the pregnancy.

Fetal pleural effusion has been reported to be associated with chromosomal abnormalities, genetic syndromes, obstructive uropathy, lymphatic vessel abnormalities such as Noonan syndrome, RASopathy and congenital lymphatic anomalies, thoracic cavity defects, Rh or ABO incompatibility, non-immune hydrops fetalis, infections, congenital cardiac anomalies, metabolic diseases and hematologic diseases such as α-thalassemia. This review provides an overview of syndromic and single gene disorders associated with fetal pleural effusion that is useful for genetic counseling and fetal therapy at prenatal diagnosis of fetal pleural effusion.

The mammalian/mechanistic target of rapamycin (mTOR) is a protein kinase that plays a crucial role in regulating cellular growth, metabolism, and survival. Although there is no absolute contraindication for the use of mTOR inhibitors during pregnancy, the specific fetal effects remain unknown. Available data from the past 2 decades have examined the use of mTOR inhibitors during pregnancy in patients with solid organ transplantation, showing no clear link to fetal complications or structural abnormalities.
