(1) Background: The aim of the present study is to analyze the instant postural changes and changes in the dystonic contractions among patients with dystonia following the use of an intraoral device called a key integrative dynamic TMJ treatment appliance (KIDTA). (2) Methods: Twelve subjects, previously diagnosed with dystonia were enrolled. Their existing records were utilized to assess the changes in their posture and dystonic contractions. The posture analysis was conducted using a mobile application (APECS). The initial records (T0) and records acquired after the delivery of the KIDTA (T1) were utilized in the analysis. The Wilcoxon signed-rank test was performed to compare parameters between T0 and T1, with a significance level set at < 0.05. (3) Results: Based on the Wilcoxon signed-rank test, statistically significant differences in T1 compared to T0 were observed in the severity of dystonic spasms, body alignment, head shift, head tilt, shoulder alignment, shoulder angle, axillae alignment, ribcage tilt, pelvic tilt, knee angle, and tibia angle ( < 0.05). (4) Conclusions: Within the limitations of the present pilot study, an intervention to the TMJ through a KIDTA appliance seems to mitigate the severity of dystonic contractions and improve the posture with respect to certain postural parameters.

Article Synopsis
  • The yips is a coordination issue in athletes, especially baseball players, tied to task-specific dystonia.
  • This study focused on two baseball players displaying different yips symptoms, analyzing their throwing motions and muscle activity using advanced technology.
  • It identified two types of dystonic movements: one involving excessive elbow flexion due to improper muscle contraction, and another featuring tremors from inconsistent muscle activity, emphasizing the need for personalized analysis in understanding and addressing yips symptoms.

DYT-THAP1 dystonia is a monogenetic form of dystonia, a movement disorder characterized by the involuntary co-contraction of agonistic and antagonistic muscles. The disease is caused by mutations in the THAP1 gene, although the precise mechanisms by which these mutations contribute to the pathophysiology of dystonia remain unclear. The incomplete penetrance of DYT-THAP1 dystonia, estimated at 40 to 60 %, suggests that an environmental trigger may be required for the manifestation of the disease in genetically predisposed individuals.

  • Oromandibular dystonia (OMD) is a condition causing muscle contractions in the jaw and related areas, and this study analyzed the effects of botulinum toxin (onabotulinumtoxinA) as a treatment.
  • A meta-analysis of 26 studies with 1103 patients showed that 96.2% experienced a positive response to the injections, with 88.9% achieving significant improvement of over 50%.
  • Although 17.8% of patients had adverse effects, mainly dysphagia (10.1%), the overall findings suggest that botulinum toxin is an effective treatment for OMD but further studies are needed for more conclusive results.

Article Synopsis
  • Tremor is a common movement disorder with various causes, and surface electromyography (SEMG) is an effective tool for diagnosing different tremor syndromes.
  • A study of 97 patients at a movement disorder clinic revealed that essential tremor was the most prevalent syndrome, with distinct electrophysiologic patterns observed across different types of tremors.
  • The results indicate that SEMG can help distinguish between tremor-ataxia syndrome and essential tremor, which can be challenging to differentiate in clinical practice.

Intermuscular coherence during co-contraction eliciting tasks as a biomarker for dystonia and GPi-DBS efficacy.

Article Synopsis
  • Clinical rating scales often miss the full range of dystonic symptoms, prompting the need for a reliable biomarker to assess the effectiveness of deep brain stimulation (GPi-DBS) for treating dystonia.
  • In a study of 12 patients undergoing GPi-DBS, researchers found that those with arm dystonia had higher intermuscular coherence in the 4-12 Hz range before surgery, which significantly decreased after the procedure.
  • Despite the observed changes in coherence, the study found no strong links between coherence levels and clinical rating scale scores, suggesting that more research is needed to determine the biomarker's clinical utility.
