During the last six years, the authors treated 44 patients for a ruptured abdominal aneurysm. Three of these patients had a spontaneous aorto-caval fistula and their clinical cases are described. The special diagnostical and therapeutical features of this rare complication are emphasized.

Article Synopsis
  • This study evaluates the outcomes of open surgical repair in 39 male patients with aorto-caval fistula caused by spontaneous rupture of an abdominal aortic aneurysm, documenting a high 30-day mortality rate of 35.9%.
  • The research involved a thorough review of clinical records at a Serbian medical center from January 2012 to February 2023, with a mean follow-up of 3.7 years for those who survived the surgery.
  • The findings suggest that given the serious nature of ACF and the risks associated with open repair, endovascular repair techniques are recommended as they may offer better outcomes.

Cardiopulmonary Resuscitation in Obstetric Patient: Special Considerations.

The prevalence of cardiac arrest in pregnant women varies from 1/20,000 to 1/50,000 pregnancies and is associated with high fetomaternal mortality. The pregnant mother is more susceptible to cardiac arrest as hypoxia is poorly tolerated. Hemorrhage, eclampsia, sepsis, and embolism are common causes of arrest.

We report a case of a 73-year-old male with multiple comorbidities, including postpoliomyelitis severe scoliosis, referred to our tertiary center due to a severe symptomatic aortic stenosis, considered high risk for surgical aortic valve replacement (AVR). Due to unsuitable femoral and subclavian accesses, the patient underwent a transcaval transcatheter AVR (TAVR) procedure, complicated by the development of an iatrogenic infrarenal aortic pseudoaneurysm with aortocaval fistula. Scoliosis can cause varying anatomic relationships between retroperitoneal vessels and intervertebral disk spaces, which increase the difficulty of the procedure and consequently lead to this vascular complication.

A rare case of paradoxical pulmonary embolism in spontaneous aortocaval fistula.

Department of Vascular Surgery, Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico, University of Milan, Milan, Italy.

Aortocaval fistula (ACF) is a rare complication of abdominal aortic aneurysm (AAA), occurring in less than 1% of all AAAs. Paradoxical embolism can rarely be associated with ACF, pulmonary embolism may originate from dislodgment of thrombotic material from the AAA in the inferior vena cava (IVC) through the ACF. We report a case of a patient admitted to the emergency department with abdominal pain and shortness of breath who immediately underwent thoraco-abdominal CT.

Endovascular aortic aneurysm repair (EVAR) is a valid treatment for patients with abdominal aortic aneurysm with aortocaval fistula. However, an endoleak can be caused by persistent communication between the aneurysm and the inferior vena cava. We present a case of impending rupture due to spontaneous obstruction of an aortocaval fistula after EVAR.
