Significance: Skin capillaries are non-invasively observable; their structure and blood flow can reflect tissue and systemic conditions. Quantitative analysis of video-capillaroscopy images yields novel diagnostic methods. Because the capillary structure is heterogeneous, analyzing more capillaries can increase the evaluation reliability.

Aim: We developed a system that can observe and quantify numerous capillaries and verified the performance on human skin.

Approach: We developed a portable video-capillaroscope with a spatial resolution higher than and a wide field of view () and a method to evaluate capillary numbers and areas using U-Net. The model was trained and tested with 22 and 11 cropped images () obtained from 11 participants, respectively. They were then applied to the images from four participants. Segmentation results were compared to ground-truth at the pixel level and capillary-region level.

Results: Over 1000 capillaries were simultaneously observed using the proposed system. Although pixel-level segmentation performance was low [intersection over union (IoU) = 24.5%], the number and area could be estimated. These values differed among four participants and seven sites, and they changed after skin barrier destruction.

Conclusions: The proposed system allows for observing and quantifying numerous skin capillaries simultaneously, suggesting its potential for evaluating tissue and systemic conditions.

Topical Carboxytherapy for Skin Rejuvenation.

J Clin Aesthet Dermatol

January 2025

Drs. Simmons-O'Brien and Orlinsky are with Simmons-O'Brien & Orlinsky, LLC, in Towson, Maryland.

Driven by public demand, new safe and effective approaches for achieving dermal rejuvenation are continuously being developed. Recently, there has been growing interest and advances in carbon dioxide therapy, or carboxytherapy. Based on the Bohr effect, carboxytherapy enhances the release of O from the blood into the surrounding tissues in response to increased blood CO.

Background: Sturge-Weber Syndrome (SWS) is a rare, sporadic neurocutaneous disorder affecting the skin, brain, and eyes, due to somatic activating mutations in GNAQ or, less commonly, GNA11 gene. It is characterized by at least two of the following features: a facial capillary malformation, leptomeningeal vascular malformation, and ocular involvement. The spectrum of clinical manifestations includes headache, seizures, stroke-like events, intellectual disability, glaucoma, facial asymmetry, gingival hyperplasia, etc.

Chronic non-healing wounds are a common complication of diabetes, marked by impaired angiogenesis. This study explores how exosomes (Exo-miR-1248) from miR-1248-overexpressing adipose-derived stem cells enhance diabetic wound healing by modulating endothelial cell function. Adipose-derived stem cells were transfected with a lentivirus carrying miR-1248 to produce Exo-miR-1248, isolated via differential centrifugation.

Introduction: The megalencephaly capillary malformation polymicrogyria (MCAP syndrome) results from mosaic gain-of-function variants. The main clinical features are macrocephaly, somatic overgrowth, neurodevelopmental delay and brain anomalies. Alpelisib (Vijoice) is a recently FDA-approved PI3Kα-specific inhibitor for patients with PIK3CA-related overgrowth spectrum (PROS).

The clinical profile and outcomes of children with chikungunya infection differ from those observed in adults. As there is a paucity of data on chikungunya infection in children, this study aimed to find the clinical course, complications, and mortality rates of chikungunya infection in children. This was a combined retrospective and prospective observational study.
