Post-menopausal bleeding (PMB) accounts for 5% of gynecologic office visits and is the presenting symptom in 90% of women with endometrial cancer, which requires prompt evaluation. The most common etiology of PMB is vaginal or endometrial atrophy and endometrial polyps, while endometrial hyperplasia and carcinoma account for less than 10% of PMB. Transvaginal ultrasonography measurement of an endometrial thickness (EMT) less than or equal to 4 mm has a 99% negative predictive value for endometrial carcinoma. Endometrial sampling is required if EMT >4 mm or persistent bleeding occurs. Further evaluation can be accomplished with saline infusion sonography, magnetic resonance imaging, and hysteroscopy.

In the present case, a 66-year-old woman presented to the Specialty Hospital (Amman, Jordan) with recurrent post-menopausal bleeding. A pelvic ultrasound scan showed an abnormal endometrial thickness of 8 mm and no adnexal masses. An endometrial biopsy revealed abundant foamy histiocyte infiltration features suggestive of xanthogranulomatous endometritis.

The preferred treatment for medically inoperable early-stage endometrial cancer (EC) is definitive radiation therapy (RT), either in the form of brachytherapy (BT) alone or in combination with external beam radiotherapy (EBRT). This case report details a modified brachytherapy approach for a 59-year-old female patient who was known to have multiple comorbidities, poor performance status, and progressive slowness in activities of daily living. She presented with a complaint of post-menopausal bleeding per vaginum, for which she underwent investigations and was diagnosed with a case of endometrial adenocarcinoma, International Federation of Gynecology and Obstetrics (FIGO) stage IA and grade I.

This paper reviews the distinct clinical, pathophysiological, and therapeutic challenges faced by older women undergoing percutaneous coronary intervention (PCI). Older women present with a greater comorbidity burden, smaller coronary vessels, and post-menopausal hormonal changes, which increase procedural complexity and adverse cardiovascular outcomes. Despite these challenges, older women are less likely to receive evidence-based therapies, resulting in higher risks of major adverse cardiovascular events (MACE) and bleeding.
