The 90-year-old female patient with underlying rheumatoid arthritis and spinal canal stenosis was admitted to hospital for pain control. The clinical exam of the lower abdomen was impressively painful and the inflammatory parameters were significantly increased. For further diagnosis a computer tomography was performed, showing a covered perforated appendicitis with a perityphlitic abscess. The conservative therapy with abscess drainage and antibiotics was successful.

We hypothesize that the lack of antibiotic prophylaxis might not be inferior to administering prophylaxis.

Bilateral preseptal cellulitis without accompanying sinusitis or skin trauma is uncommon. In this report, we present a case of bilateral preseptal cellulitis and an upper eyelid abscess in an otherwise healthy child. A nine-year-old girl presented with severe and progressive bilateral swelling of the upper lids that showed an unsatisfactory response to medical treatments (intravenous ceftazidime and vancomycin) and warranted a referral to our facility.
