A 69-year-old man with prostate cancer treated with radical prostatectomy underwent 68 Ga-PSMA PET/CT due to biochemical recurrence. Increased radiotracer uptake in the prostate bed suggestive of local recurrence was detected. The right hip also displayed increased 68 Ga-PSMA activity, which was consistent with the avascular necrosis findings on MRI.

Although specific techniques vary, core decompression is generally accepted as the treatment of choice for precollapse avascular necrosis (AVN) of the hip to delay or prevent progression of the disease. Bone marrow edema (BME), a notable manifestation during the progression of osteonecrosis of the femoral head (ONFH), exhibits significant associations with femoral head collapse, pain, and prognosis, howeverits' pathogenesis remains underexplored. In this study, specimens from patients undergoing total hip arthroplasty (THA) were analyzed. The results revealed significantly higher Visual Analog Scale (VAS) scores and CT low-density area ratio in the BME group compared to the control group.
