AI Article Synopsis

  • A 14-year-old boy had an oblique fracture in the lower part of his shin (tibia) that didn't heal properly without surgery, partly due to the anterior tibial tendon getting caught on a bony spike from the broken bone.
  • A 3D CT scan helped doctors identify the issue before surgery.
  • Surgery successfully released the tendon, fixed the fracture, and restored full, pain-free function to the leg.

Article Abstract

Case: An oblique fracture of the distal third of the tibia, treated nonoperatively in a 14-year-old adolescent boy, did not unite because of ensnarement of the anterior tibial tendon (ATT) around an anterior inferior bony spike from the proximal tibial fragment. Computed tomography scan with 3-dimensional volume rendering aided in preoperative diagnosis. Surgical extraction of the tendon from within the fracture site and internal fixation led to successful union and full painless function.

Conclusion: Beware of possible ATT entrapment as a cause of irreducibility of oblique distal third tibial fractures with an anterior inferior bony spike of the proximal fragment.

Background/purpose: Autologous dentin materials are among the most promising bone substitutes for preventing osseous defects on the distal side of the lower second molar. This study aimed to investigate the effects of autologous demineralized dentin matrix on postoperative complications and wound healing after lower third molar surgery.

Materials And Methods: Thirteen patients with bilateral symmetrical lower third molars participated in this split-mouth randomized clinical trial.

Aminoglycosides are concentration-dependent antibiotics exerting a bactericidal effect when concentrations at the site of infection are equal to or greater than 5 times the minimum inhibitory concentrations (MIC). When administered intravenously, they exhibit poor lung penetration and high systemic renal and ototoxicity, imposing to restrict their administration to 5 days. Experimental studies conducted in anesthetized and mechanically ventilated sheep and pigs provide evidence that high doses of nebulized aminoglycosides induce a rapid and potent bacterial killing in the infected lung parenchyma.

Background: Coronary sinus (CS) lead placement in persistent left superior vena cava (PLSVC) cases is challenging because of the poor backup force of the guiding catheter within the enlarged CS. Active fixation Quadripolar leads (Attain Stability™ Quad 4798, Medtronic) can expand choice to CS branches with limited access; however, no cases of anchoring to the main body of the CS have been published to date.

Case Summary: We describe a case of cardiac resynchronization therapy pacemaker upgrade in a 79-year-old female who developed pacing-induced cardiomyopathy after pacemaker implantation via the right superior vena cava (SVC) for atrioventricular block eight years ago wherein PLSVC was revealed during the procedure.

Irreducible ankle fracture-dislocations due to posterior tibialis tendon (PTT) interposition in the distal tibiofibular and tibiotalar joints are rarely reported, and their diagnoses are often missed and delayed. In addition, previous reports lacked a longer clinical follow-up period, and objective and subjective evaluations of such cases have not been reported. A 22-year-old man sustained an open fracture-dislocation of the ankle joint associated with a distal third of the fibular shaft fracture and diastasis of the distal tibiofibular joint.

Introduction: Organ preservation (OP) strategies are gaining interest in improving the quality of life in the management of rectal cancer, particularly for tumors located in the distal or middle rectum. The optimal OP protocol is still not standardized and relies on randomized trials. This review summarizes past and ongoing studies on OP protocols for adenocarcinoma of the distal and middle rectum.
