Objective: This study aims to assess the efficacy of anchored sutures (AS) in securing the anterolateral thigh (ALT) flap during oral and oropharyngeal reconstructions, and its impact on the occurrence of orocutaneous fistula (OCF).

Materials And Methods: A retrospective study was performed on patients who underwent ALT flap reconstruction in our department in the year 2022. The patients were divided into two groups based on whether the AS technique was used. The incidence of OCF was compared between the two groups, and AS-related complications were reported. Fisher's exact test was employed to assess the differences in baseline characteristics and the incidence of OCF between the two groups.

Results: The study included 214 patients, with 156 in the conventional suture (CS) group and 58 in the AS group. The incidence of OCF in the AS group was significantly lower compared to that in the CS group (P = 0.039). However, there was a weak correlation between OCF and the AS technique (φ = -0.149). Among the 58 cases in the AS group, three (5.17%) experienced AS-related granuloma (ASRG) as complications.

Conclusion: The use of ALT flap reconstruction with the AS technique reduces the incidence of OCF; however, ASRG may be a potential complication.

Clinical Relevance: This study demonstrates the effectiveness of AS technique in securing ALT flaps, leading to a decreased risk of OCF in oral and oropharyngeal defect reconstruction.

Occipital condyle fractures revisited.

Emerg Radiol

December 2024

Russell H. Morgan Department of Radiology and Radiological Science, Johns Hopkins Medical Institution, 600 N. Wolfe Street, Phipps B100, Baltimore, MD, 21287, USA.

Purpose: Occipital condyle fractures (OCFs) are classified by the Anderson and Montesano system into Type I (comminuted, minimally displaced), Type II (stable, associated with basilar skull fractures), and Type III (unstable avulsion fractures). We retrospectively analyzed 24,986 cervical spine CT examinations of emergency department patients over five years to determine the incidence and characteristics of OCFs, mechanism of injury, and associated intracranial and cervical spine injuries.

Methods And Materials: The study was IRB-approved and HIPAA compliant.

Treatment of relapsed/refractory multiple myeloma (RRMM) is challenging as patients exhaust all available therapies and the disease becomes refractory to standard drug classes. Here we report the final results of LocoMMotion, the first prospective study of real-world clinical practice (RWCP) in triple-class exposed (TCE) patients with RRMM, with a median follow-up of 26.4 months (range, 0.

[Osteochondral fractures in childhood and adolescence].

Orthopadie (Heidelb)

September 2024

Klinik für Unfallchirurgie, Orthopädie und Plastische Chirurgie, Universitätsmedizin Göttingen, Robert-Koch-Str. 40, 37075, Göttingen, Deutschland.

Article Synopsis
  • Osteochondral fractures (OCFs) are injuries involving both cartilage and bone, often caused by patellar dislocation, with common sites being the medial patellar facet and lateral femoral condyle, affecting all age groups.
  • Diagnosis typically includes identifying a painful knee effusion and limited movement, with X-rays and MRI recommended for further assessment, especially when there's bleeding in the joint.
  • Treatment aims to restore joint alignment and may involve conservative methods for small fractures, while surgery is needed for more severe cases, utilizing techniques like refixation and cartilage regeneration to lower the risk of long-term osteoarthritis.*

Background: Although the incidence of double primary cancers (DPCs) involving lung cancer is rising, they have not been studied sufficiently. This study retrospectively analyzed the clinicopathological and prognostic characteristics of DPC patients with lung cancer and developed a survival nomogram to predict the individual OS rates.

Methods: We included 103 DPC patients with lung cancer from Shengjing Hospital between 2016 and 2021.

Article Synopsis
  • The study aimed to investigate how congenital anomalies at the atlanto-occipital joint affect the stability of the craniovertebral junction (CVJ) in children with Chiari malformation type I and syringomyelia, focusing on the prevalence of certain anomalies and their connection to occipitocervical fusion (OCF) after surgery.
  • Researchers analyzed data from patients in the Park-Reeves Syringomyelia Research Consortium, comparing those who underwent posterior fossa decompression with OCF to those who only had posterior fossa decompression, while ensuring both groups were similar in age, sex, and symptoms.
  • Results indicated that the group which underwent both procedures had significantly higher angles
