AI Article Synopsis

  • A new spinal immobilization protocol in the Netherlands raised concerns about increased non-immobilized spinal fractures (NISFs) and worse patient outcomes.
  • A pilot study analyzed trauma patient records from two periods (2013-2014 with strict protocol versus 2017-2018 with selective protocol) to evaluate adverse outcomes and assess if a larger study was viable.
  • Results showed no cases of neurological injury or mortality linked to NISFs in either period, suggesting that a larger study is unnecessary; future research should instead identify which patients benefit from spinal immobilization.

Article Abstract

Background: A new selective preventive spinal immobilization (PSI) protocol was introduced in the Netherlands. This may have led to an increase in non-immobilized spinal fractures (NISFs) and consequently adverse patient outcomes.

Aim: A pilot study was conducted to describe the adverse patient outcomes in NISF of the PSI protocol change and assess the feasibility of a larger effect study.

Methods: Retrospective comparative cohort pilot study including records of trauma patients with a presumed spinal injury who were presented at the emergency department of a level 2 trauma center by the emergency medical service (EMS). The pre-period 2013-2014 (strict PSI protocol), was compared to the post-period 2017-2018 (selective PSI protocol). Primary outcomes were the percentage of records with a NISF who had an adverse patient outcome such as neurological injuries and mortality before and after the protocol change. Secondary outcomes were the sample size calculation for a larger study and the feasibility of data collection.

Results: 1,147 records were included; 442 pre-period, and 705 post-period. The NISF-prevalence was 10% (95% CI 7-16, n = 19) and 8% (95% CI 6-11, n = 33), respectively. In both periods, no neurological injuries or mortality due to NISF were found, by which calculating a sample size is impossible. Data collection showed to be feasible.

Conclusions: No neurological injuries or mortality due to NISF were found in a strict and a selective PSI protocol. Therefore, a larger study is discouraged. Future studies should focus on which patients really profit from PSI and which patients do not.

The impeding ban on per- and polyfluoroalkyl substances (PFAS) prompted researchers to focus on hydrocarbon-based materials as constituents of next-generation proton exchange membranes (PEMs) for polymer electrolyte fuel cells (PEFCs). Here, we report on the fuel cell performance and durability of fluorine-lean PEMs prepared by the post-sulfonation of co-grafted α-methylstyrene (AMS) and 2-methylene glutaronitrile (MGN) monomers into preirradiated 12 µm polyvinylidene fluoride (PVDF) base film. The membranes were subjected to two distinctly different accelerated stress test (AST) protocols performed at open-circuit voltage (OCV): the US Department of Energy-similar chemical AST (90 °C, 30% relative humidity (RH), H/air, 1 bar), developed originally for perfluoroalkylsulfonic acid (PFSA) membranes, and the high relative humidity AST (80 °C, 100% RH, H/O, 2.

Diffusion of mobile charge carriers, such as ferredoxin and plastocyanin, often constitutes a rate-determining step in photosynthetic energy conversion. The diffusion time scales typically exceed that of other primary bioenergetic processes and remain beyond the reach of direct simulation at the molecular level. We characterize the diffusive kinetics of ferredoxin and plastocyanin upon the photosystem I-rich domain of , the most abundant phototroph on Earth by mass.

Irritability in stroke: a protocol for a prospective study.

Front Neurol

December 2024

Department of Medicine and Therapeutics, The Chinese University of Hong Kong, Hong Kong, Hong Kong SAR, China.

Background: Poststroke irritability (PSI) is common among stroke survivors and can lead to a poor quality of life, difficulties in social interactions, criticism from caregivers, and caregiver stress. The planned study will evaluate the clinical, neuropsychological, and magnetic resonance imaging (MRI) correlates of PSI in a cohort of stroke survivors. In addition, the study will examine the 15-month progression of PSI.

Unlabelled: The rate of exposure to traumatic events is high globally, and post-traumatic stress disorder (PTSD) is one of the most serious consequences. It was reported that approximately 14 million people were affected by earthquakes in Turkey in February 2023. Cognitive Behavioral Therapy (CBT) is a therapeutic approach to treating PTSD and has a proven efficacy.

Article Synopsis
  • A study examined the effectiveness of various assistive techniques in total knee arthroplasty (TKA), including conventional instruments, computer-assisted systems, patient-specific instruments, and robotic systems, through a network meta-analysis of randomized controlled trials.
  • The analysis, which included 112 trials and nearly 15,000 TKAs, found that robotic systems (RAS) were the most accurate for mechanical alignment, while computer-assisted systems (CAS) performed best for pain relief, and patient-specific instruments (PSI) had the shortest operation time.
  • Ultimately, while RAS showed the best alignment accuracy, there were no significant differences in clinical outcomes or complications among the four techniques.
