Sarcomas represent 1% of malignancies in adult population; thereby dermatofibrosarcoma protuberans is found in 1%-2% of all cases. A surgical approach in oncologic treatment is the standard of care; therefore, important is an extensive resection to achieve clear margins and prevent recurrence. Herein we report a case of recurrent dermatofibrosarcoma protuberans in the thoracolumbar region. As the adequate resection was to cause a huge defect, we have made a reconstruction plan for coverage, using a perforator-based double Keystone Island flap (Type III). The method was chosen as a reliant solution with low complication rates and without need for complex flap designs thereby giving good functional and esthetic results. Main aim of the case report was to show that Keystone Island flaps present a simple and technically straightforward method with low complication rates and good results.

Anthropogenic planetary heating is disrupting global alpine systems, but our ability to empirically measure and predict responses in alpine species distributions is impaired by a lack of comprehensive data and technical limitations. We conducted a comprehensive, semi-quantitative review of empirical studies on contemporary range shifts in alpine insects driven by climate heating, drawing attention to methodological issues and potential biotic and abiotic factors influencing variation in responses. We highlight case studies showing how range dynamics may affect standing genetic variation and adaptive potential, and discuss how data integration frameworks can improve forecasts.

The keystone design perforator island flap (KDPIF) was first described by Behan in 2003 as a pliable flap, suitable for all body areas thanks to its curvilinear shape, which made it apt to fit mostly all cutaneous defects. In this article, we aim to share our experience using KDPIF for the reconstruction of different defects. A retrospective analysis of patients who underwent cutaneous excision after trauma or tumor resection and reconstruction with a KDPIF at our department from 2020 to 2023 was performed.

Quaternary ammonium compounds (QACs), ecotoxic organic chemicals linked to multidrug resistance, are being used increasingly, for example to prevent the transmission of infections such as covid-19, in households, hospitals, and industry. To understand the locations, fluctuations, and fractions of QACs entering sewers, we monitored 14 QACs (benzalkonium chloride [BAC]-C8, C10, C12, C14, C16, and C18; dialkyldimethylammonium chloride [DDAC]-C8, C10, and C12; alkyltrimethylammonium chloride [ATAC]-C12, C16, and C18; benzethonium chloride; and cetylpyridinium chloride), and a disinfectant (chlorhexidine) in influent at four Japanese sewage treatment plants (STPs) five times throughout a year. Mass inflows were relatively stable throughout the year, indicating that the recent seasonal covid-19 epidemic did not greatly influence them.

To investigate the effect of Keystone Design Perforator Island Flap (KDPIF) in the repair of maxillofacial soft tissue defect. From June 2021 to June 2023, 16 patients with soft tissue defects caused by resection of maxillofacial tumor repaired by KDPIF were selected. Variants of KDPIF were designed according to the area of the defect, and the incision was parallel to the wrinkle as far as possible in the same facial cosmetic subunit.
