A 66-year-old man who developed an occluded left main pulmonary artery, clinically demonstrated by perfusion scan and pulmonary angiography, is described. Explorative thoracotomy revealed a granulomatous giant cell arteritis after pathologic examination of the surgically removed left lung. Detailed clinical and laboratory evaluation excluded a systemic disease.

Sarcoidosis is a multisystem granulomatous disease of unknown etiology, characterized by noncaseating epithelioid granuloma, multinucleate giant cells, and tissue destruction. While lung and lymph node involvement is common, isolated renal involvement is rare. We report the case of a 55-year-old female patient, with renal limited sarcoidosis, who presented with worsening sensorium and acute kidney injury.

Miliarias are a group of eccrine disorders characterized by sweat retention due to the occlusion of eccrine ducts. Miliaria profunda is the rarest form of miliaria and occurs when eccrine ducts are obstructed at the dermal-epidermal junction. A granulomatous variant of centrifugal miliaria profunda classic findings of granulomatous infiltrate on histology and centrifugal expansion of deep nodules or plaques.

Objectives: GCA is a granulomatous vasculitis affecting large vessels, leading to intimal occlusion accompanied by the accumulation of myofibroblasts. Histopathologically, GCA is characterized by destruction of the tunica media and hypertrophy of the intima with invasion of activated CD4+ T cells, macrophages and multinucleated giant cells (MNGCs). Despite these well-defined histopathological features, the molecular pathology of GCA has largely remained elusive.
