Background: Direct-to-implant breast reconstruction is a standard method of implant-based reconstruction; however, the risk of skin necrosis and implant failure exists. During simultaneous contralateral augmentation, an implant larger than the resected tissue must be inserted to balance both sides, which may increase the risk of complications. This study examined the differences in the incidence of complications between the single-stage direct-to-implant reconstruction and 2-stage reconstruction when contralateral augmentation was performed simultaneously.
Methods: This study included 99 patients who underwent implant-based breast reconstruction with contralateral augmentation between 2012 and 2020. A retrospective chart review was conducted and the patients were divided into 2 groups: the single-stage reconstruction (n = 61) and 2-stage reconstruction (TSR, n = 38) groups. Demographic data and surgical and oncological information were collected. Complications including skin necrosis and reconstruction failure were investigated as the primary outcome.
Results: Implant volume and contralateral implant volume were significantly greater in TSR group than in the single-stage reconstruction group. There were no significant differences in the incidence of complications and reconstruction failure rates between both groups. The nipple-sparing mastectomy was the risk factor for complications. Furthermore, the risk factors for mastectomy skin necrosis were implant volume and differences in the volume of both implants.
Conclusions: Single-stage reconstruction did not increase the risk of complications compared with TSR when implants that were larger than the resected tissue were inserted after mastectomy. Proper patient selection and selection of implants that are not excessively large could satisfy patients' cosmetic needs in a single operation.

The unique saddle articulation of the trapeziometacarpal joint allows for a wide range of motion necessary for routine function of the thumb. Inherently unstable characteristics of the joint can lead painful instability. In this study, we modified a surgical dorsal ligament reconstruction technique for restoring trapeziometacarpal joint stability.

To address the challenges related to active power dissipation and node voltage fluctuation in the practical transformation of power grids in the field of new energy such as wind and photovoltaic power generation, an improved Dung Beetle Optimization Algorithm Based on a Hybrid Strategy of Levy Flight and Differential Evolution (LDEDBO) is proposed. This paper systematically addresses this issue from three aspects: firstly, optimizing the DBO algorithm using Chebyshev chaotic mapping, Levy flight strategy, and differential evolution algorithm; secondly, validating the algorithm's feasibility through real-time network reconfiguration at random time points within a 24-h period; and finally, applying the LDEDBO to address the dynamic reconfiguration problems of the IEEE-33 and IEEE-69 node bus. The simulation indicates that the power dissipation of the IEEE-33 node bus is decreased by 28.

Background: Congenital hypoplasia of the depressor labii inferioris muscle can lead to Asymmetrical crying facies(ACF). The objective of this research was to examine the alterations in both static and dynamic facial symmetry following the resection of the depressor labii inferioris on the healthy side through an intraoral approach.
Methods: Patients exhibiting palsy of the depressor labii inferioris muscles were included in the study.

Introduction: Continued interest in the optimization of recovery in aesthetics has led to the exploration of adjunctive therapies. Hyperbaric oxygen therapy (HBOT) serves as one such therapy that may have an impact in this field. HBOT is hypothesized to improve ischemia, reduce swelling, and minimize secondary hypoxic tissue damage.
