Sexually transmitted proctitis, a prevalent concern among men who have sex with men (MSM) is frequently caused by a range of pathogens, including herpes simplex virus (HSV), and While HSV-associated proctitis typically presents with visible lesions, cases without external manifestations remain evasive. We report the case of an MSM in his early 30s presenting with dyschezia and perineal discomfort after unprotected anoreceptive intercourse. Despite initial inconspicuous findings, rectal swabs revealed HSV-2 infection.

Proctitis refers to inflammation in the rectum and may result in rectal bleeding, discharge, urgency, tenesmus, and lower abdominal pain. It is a common presentation, particularly in genitourinary medicine and gastroenterology, as the two most common causes are sexually transmitted infections and inflammatory bowel disease. The incidence of infective proctitis is rising, particularly amongst high-risk groups, including men who have sex with men, those with HIV seropositive status, and those participating in high-risk sexual behaviours.

Article Synopsis
  • The study aimed to assess the occurrence and features of concurrent bacterial sexually transmitted infections (bSTIs) in individuals diagnosed with mpox in Rio de Janeiro, Brazil.
  • Out of 634 participants, 538 were tested for STIs; the overall rate of bSTIs was found to be 37.3%, primarily affecting cisgender men aged 30-39, with many also living with HIV.
  • The findings highlight the significant coexistence of bSTIs in mpox patients, suggesting a need for better sexual health strategies that include mpox in diagnoses and preventive measures.

June 2024

Public Health National Program Office, Department of Veterans Affairs, Washington, DC 20420, USA.

Article Synopsis
  • A study of 1381 HIV-positive Veterans from 2016 to 2023 found that 20.6% met the clinical criteria for lymphogranuloma venereum (LGV), with 63% classified as probable cases.
  • Of the identified LGV cases, 81% presented with proctitis, and key risk factors included male birth sex and being a man who has sex with men.
  • Only 43.7% of the LGV cases received standard treatment, suggesting that LGV may be underestimated and inadequately treated in this population.

Article Synopsis
  • Lymphogranuloma venereum (LGV) is a sexually transmitted infection primarily affecting men who have sex with men (MSM), with significant prevalence in HIV-positive individuals and proctitis as a common symptom.
  • A study was conducted to analyze the genetic characteristics of LGV isolates from six hospitals in Spain using multilocus sequence typing (MLST) and other genetic methods.
  • Results showed that a majority of isolates were L2 genotypes, with L2-ST53 and L2-ST58 being the most common profiles, indicating complex transmission networks and potential co-circulation of different strains.
