A physically independent and mentally competent 61-year-old gentleman was diagnosed with left upper alveolar spindle cell carcinoma. He underwent left infrastructure maxillectomy with reconstruction and had bismuth iodoform paraffin paste packed in the post-operative cavities. The patient subsequently exhibited labile emotions and acute delirium. Further investigation showed that his urine iodine level was highly elevated. He eventually made a full recovery and returned home.

Background: Enhanced iodide intake in NOD.H2(h4) mice accelerates the incidence and severity of spontaneous autoimmune thyroiditis (SAT) via an unknown mechanism. A plausible hypothesis is that iodide-induced apoptosis of thyrocytes can create imbalances in antigenic load and/or disruption of immunoregulatory mechanisms that facilitate activation of autoreactive T cells in cervical lymph nodes draining the thyroid.

We describe the occurrence of monolateral aseptic sialadenitis following non-iodinated contrast medium (ICM) administration for a carotid artery stenting procedure in a 71-year-old man. The mechanism for iodide-induced sialadenitis may be idiosyncratic or related to toxic accumulation of iodide. The risk for sialadenitis is directly related to serum iodide levels (> 10 mg/100 mL) and inversely related to normal renal function so that in renal impairment, ICM can be eliminated through alternative pathways such as the salivary glands and other excretory organs.
