Recent concerns regarding the clinical accuracy of pulse oximetry in dark-skinned patients, specifically in detecting occult hypoxaemia, have motivated research on this topic and recently reported in this journal. We provide an overview of the technical aspects of the issue, the sources of inaccuracy, and the current regulations and limitations. These insights offer perspectives on how pulse oximetry can be improved to address these potential limitations.

To reduce the risk of syncope, trained breath-hold divers (BHDs) use a specialized breathing technique after surfacing called "hook breathing" (HB). It consists of a full inspiration followed by a Valsalva-like maneuver and with subsequent exhalation performed against resistance to generate continuous positive airway pressure during exhalation. Brachycephalic breeds suffer from respiratory distress known as brachycephalic obstructive airway syndrome (BOAS) and the multiple comorbidities associated with it. Targeted breeding toward a more BOAS-free phenotype requires accurate and least invasive detection of BOAS severity grades that are accessible and accepted by the breeders and kennel clubs. Intermediate care (IC) is prevalent nationwide, but little is known about how to best organize this level of care. Using a 99-item cross-sectional survey assessing four domains (hospital and physical IC features, provider and nurse staffing, monitoring, and interventions/services), we describe the organizational heterogeneity of IC within a five-hospital healthcare system. Surveys were completed by nurse managers from 12 (86%) of 14 IC settings.
