Purpose Of Review: Lumen apposing metal stents (LAMS) have been increasingly used to manage patients with surgically altered anatomy (SAA), who would have otherwise required percutaneous or surgical interventions. Via the creation of de-novo anastomoses, LAMS provides a conduit to access distal parts of the gastrointestinal tract to perform various interventions.
Recent Findings: Pancreatobiliary (PB) and non-PB interventions are challenging in patients with SAA. Endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography (ERCP) (EDGE) and endoscopic ultrasound (EUS)-directed transenteric ERCP (EDEE) are effective procedures that are being increasingly utilized to perform ERCP in patient with Roux-en-Y gastric bypass (RYGB) and non-RYGB SAA, respectively. Furthermore, EUS-directed trans-gastric intervention (EDGI) is a collective term for the non-PB procedures that could be performed in patients with SAA. EDGE/EDEE/EDGI are considered relatively safe procedures, however, intra-procedural stent misdeployement, and long-term ramifications of persistent fistula and stent migration could require endoscopic or, in rare instances, surgical management.
Summary: The advent of LAMS has revolutionized the therapeutic capabilities of EUS, by allowing the creation of new gastrointestinal anastomoses. With the growing body of literature, it is expected that such techniques will be more commonly performed in the community, providing less invasive and more effective treatment options for patients with SAA.

Combination of immunosuppressive therapy (IST) and thrombopoietin receptor agonists has revolutionized the treatment of aplastic anemia. In this study, 18 patients with eltrombopag (EPAG)-refractory and intolerant AA, including 16 with severe AA, were switched from EPAG to romiplostim and continued with romiplostim for at least three months. Of the 18 patients (7 refractory and 11 intolerant to EPAG), 13 (72%) achieved a response, with a therapeutic response in at least one lineage within three months, and nine patients (50%) showed a trilineage response.

Background: Older adults with frailty have high risk for poor postoperative outcomes.
Objective: To evaluate the feasibility of a multimodal prehabilitation program in older adults with frailty awaiting hip or knee replacement.
Design: Parallel two-arm randomized controlled pilot trial.
Chronic lymphedema is a progressive, disfiguring disease that results from dysfunction of the lymphatic vasculature, causing distal accumulation of interstitial fluid, localized development of tissue edema, and expansion of subcutaneous adipose tissue (SAT). As the molecular mechanisms governing SAT remodeling in this disease are unclear, we performed single-nucleus RNA sequencing on paired control and affected SAT biopsies from patients with unilateral lymphedema. Lymphedema samples were characterized by expansion of SAA adipocytes, pro-adipogenic stem cells, and proliferation of lymphatic capillaries.

The accurate diagnosis of synucleinopathies-neurodegenerative diseases marked by misfolded α-synuclein protein aggregates, such as Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy-remains a critical challenge. Conventional clinical criteria, frequently verified only through postmortem examination, results in diagnostic delays that impede timely intervention. Seeding amplification assay (SAA) has emerged as a promising diagnostic tool, offering high sensitivity for detecting α-synuclein aggregates even in early disease stages.
