However, many countries, including Rwanda, have documented negative experiences during childbirth. The twin reversed arterial perfusion (TRAP) sequence is a rare complication associated with monochorionic twins. It is characterized by blood flow from the umbilical artery of the normal (pump) twin to the umbilical artery of the abnormal (acardiac) twin via artery-to-artery anastomosis. This condition is associated with 100% mortality in the acardiac twin and a high rate of perinatal morbidity and mortality in the pump twin, primarily due to intrauterine hypoxic injury, heart failure, and prematurity.

We aimed to develop and evaluate Explainable Artificial Intelligence (XAI) for fetal ultrasound using actionable concepts as feedback to end-users, using a prospective cross-center, multi-level approach. We developed, implemented, and tested a deep-learning model for fetal growth scans using both retrospective and prospective data. We used a modified Progressive Concept Bottleneck Model with pre-established clinical concepts as explanations (feedback on image optimization and presence of anatomical landmarks) as well as segmentations (outlining anatomical landmarks).

Background: Induction of labour (IOL) is a common obstetric intervention in the UK, affecting up to 33% of deliveries. IOL aims to achieve a vaginal delivery prior to spontaneous onset of labour to prevent harm from ongoing pregnancy complications and is known to prevent stillbirths and reduce neonatal intensive care unit admissions. However, IOL doesn't come without risk and overall, 20% of mothers having an induction will still require a caesarean section birth and in primiparous mothers this rate is even higher.
