Objective: The authors report the first case of intracranial fibromuscular dysplasia (FMD) presenting with rapid formation of an unruptured large vertebral artery (VA) fusiform aneurysm.

Case Presentation: A 41-year-old man presented with left hemiparesis. He had a giant thrombosed aneurysm at the basilar artery-superior cerebellar artery (BA-SCA) junction and a left extracranial VA aneurysm. A VA fusiform aneurysm developed during a 1-month interval following the first session of intravascular coil embolization for the BA-SCA aneurysm. Stress on the fragile artery due to FMD during micro-catheterization may have caused the aneurysm. An anomalous aortic origin of the left VA may also have played a role in the formation of the large aneurysm and extracranial VA aneurysm. We performed overlapping stent-assisted coil embolization for the VA fusiform aneurysm using an Enterprise VRDs, and coil embolization for the BA-SCA thrombosed aneurysm using the stent-assisted technique with an Enterprise stent. Both aneurysms remained occluded for 7 years.

Conclusion: We concluded micro-catheterization to be the cause of the large aneurysm. This case emphasizes the importance of carefully performing intravascular interventional procedures for patients with FMD.

Background: The contact aspiration (CA) technique is often used to perform endovascular thrombectomy (EVT) for acute ischemic stroke (AIS); however, rescue strategies are necessary if CA fails to achieve recanalization. This study investigates the outcomes of incorporating stent retriever (SR) thrombectomy in the rescue strategy following failed CA.

Methods: EVT patients with failed CA attempts were identified from a large multicenter registry and stratified by rescue technique: CA alone or incorporating SR in the rescue strategy.

Rationale And Objectives: Individuals with autosomal dominant polycystic kidney disease (ADPKD) can present with diverse renal and extra-renal manifestations. Large vessel anomalies, such as cerebral aneurysms, are potentially fatal extra-renal manifestations. However, limited research has been conducted on cerebral small vessel disease (CSVD).

: This study aimed to evaluate whether gender-related anatomical differences in proximal aortic neck morphology affect the feasibility and outcomes of endovascular aortic aneurysm repair (EVAR) in women with abdominal aortic aneurysms (AAA). : This study performed a retrospective analysis of patients electively treated by EVAR for infrarenal AAA between January 2019 and December 2023. Demographics, anatomical characteristics, and stent graft details were analyzed.

Background: The elderly population (≥80 years) were underrepresented in recent trials of endovascular thrombectomy (EVT) for anterior circulation large vessel occlusion acute ischemic stroke (LVO-AIS) with low Alberta Stroke Program Early CT Score (ASPECTS) (≤5).

Methods: This study analyzed data from a prospectively maintained database of 37 thrombectomy centers. The primary cohort of the study comprised patients with LVO-AIS aged ≥80 who underwent EVT with ASPECTS≤5 from 2013 to 2023.
