Changes in central hemodynamics in patients suffering from acute macrofocal and transmural myocardial infarction with or without circulatory insufficiency were studied by radiocardiography. Cardiac output was found to be increased in the first 24 hours of the disease in persons without circulatory insufficiency as compared to the control group, which may be linked with marked activation of the sympatheticoadrenal system. In the groups of patients with circulatory insufficiency the cardiac output during this period was diminished. In most patients myocardial infarction was preceded by marked changes in teh myocardium. Diminution of hemodynamic values was recorded in all groups of patients on the third day, which was most conspicuous in cases with circulatory insufficiency.

Primary graft dysfunction (PGD) is the most common cause of early mortality following heart transplantation. Although PGD can affect both ventricles, isolated right ventricular dysfunction (RV-PGD) is observed in nearly half of PGD patients. RV-PGD requires specific medical management to support the preload, afterload, and function of the failing RV; however, the use of mechanical circulatory support of the RV (RV-MCS) might be required when optimal medical therapy is insufficient in preventing forward failure and retrograde venous congestion.

Background: Despite successful recanalization via mechanical thrombectomy (MT), only half of acute ischemic stroke (AIS) patients achieve functional independence. Post-MT hemodynamic features are insufficiently investigated, and the lack of bilateral comparisons limits individual assessment. Therefore, we aimed to explore how individualized hemodynamic features affect functional outcomes using quantitative digital subtraction angiography (Q-DSA).

ECMO is an advanced technology for extracorporeal respiratory and circulatory support. It involves the extraction of venous blood from the patient's body, which is subsequently oxygenated within an oxygenator (or membrane lung). This oxygen-rich blood is reinfused either into veins or arteries, rapidly compensating for impaired lung and heart functionalities.

Regulation of the blood-brain barrier function by peripheral cues in health and disease.

Institute of Neurology (Edinger Institute), University Hospital, Goethe University Frankfurt, Frankfurt am Main, Germany.

The blood-brain barrier (BBB) is formed by microvascular endothelial cells which are ensembled with pericytes, astrocytes, microglia and neurons in the neurovascular unit (NVU) that is crucial for neuronal function. Given that the NVU and the BBB are highly dynamic and regulated structures, their integrity is continuously challenged by intrinsic and extrinsic factors. Herein, factors from peripheral organs such as gonadal and adrenal hormones may influence vascular function also in CNS endothelial cells in a sex- and age-dependent manner.

Article Synopsis
  • - The case report discusses a rare occurrence of total reversal of pulmonary circulation (RPC) in a 56-year-old man who was on veno-arterial extracorporeal membrane oxygenation (V-A ECMO) during emergency cardiopulmonary resuscitation after a heart attack and cardiac arrest.
  • - The patient's previously undiagnosed aortic and mitral valve leaks, combined with the effects of V-A ECMO, led to severe pulmonary edema, a condition where fluid accumulates in the lungs.
  • - By modifying the ECMO setup to veno-pulmonary arterial-arterial (VPa-A) ECMO and cannulating the pulmonary artery, medical staff were able to relieve the pulmonary edema, but this
