We present a case of antidromic AVRT involving an atriofascicular pathway in a patient with an atretic coronary sinus os. This patient had left ventricular dysfunction with rate-related left bundle branch block aberrancy in sinus rhythm. In the electrophysiology lab, there was VA dissociation for a few beats during antidromic tachycardia. The possible mechanisms that could explain this intracardiac finding are discussed.

Article Synopsis
  • A 44-year-old male experienced symptomatic supraventricular tachycardia (SVT) with a wide QRS complex and underwent an electrophysiological study to diagnose the condition.
  • The SVT was identified as antidromic atrioventricular reentrant tachycardia (AVRT) involving an accessory pathway, with a noticeable change in QRS morphology during the episode before it resolved spontaneously.
  • Successful mapping and radiofrequency treatment eliminated the accessory pathway, making the tachycardia episodes unable to occur again, marking a unique case in clinical findings.

Multiple accessory pathways (APs) can develop in patients with Ebstein anomaly. Rarely, these APs can participate in antidromic atrioventricular reentrant tachycardia (AVRT) which can be life-threatening and requires unique considerations for acute management and ultimate ablation. These considerations are discussed herein.

Open window mapping for redo accessory pathway ablation in Ebstein anomaly.

Accessory pathway ablation in Ebstein anomaly can be significantly more challenging than in structurally normal hearts. An alternative to the conventional approach to mapping APs is to detect points with a high-density mapping catheter based on an automated detection algorithm using open window mapping. It detects the sharpest signal at each point with high-density mapping rather than relying on the origin of the local electrogram to localize the pathway and determine a site for successful ablation.

Introduction: We report the case of a 41-year-old female with documented narrow QRS tachycardia. During electrophysiological study, both orthodromic and antidromic atrioventricular reentry tachycardia (AVRT) were demonstrated as well as short episodes of pre-excited atrial fibrillation. Programmed atrial stimulation resulted in decremental anterograde conduction on the AP, thus confirming an unexpected Mahaim accessory pathway (AP) diagnosis.
