Unlabelled: Detection of hepatic metastasis from colorectal cancer remains a clinical challenge. In this case report, we present a 66-year-old male patient with a rectal carcinoma who underwent F-FDG PET-CT for staging, which revealed one hepatic metastasis. Abdominal magnetic resonance imaging (MRI) with diffusion-weighted sequence with a b value of 1000 was performed, which identified a second metastasis of the liver.
Teaching Point: This case report illustrates that, in some patients, diffusion-weighted MRI with a b value of 1000 might be a more sensitive technique for detecting small hepatic metastases than F-FDG PET-CT.

Methods: 347 patients with liver metastases (180 from GI and 167 from non-GI) and abdominal CECT including arterial, portal venous, and delayed phases were divided into training (221) and validation (96) sets at a ratio of 7:3 and an independent testing set (30).

Liver and lung are the most common metastatic sites in colorectal cancer (CRC), where the tumor microenvironment (TME) plays a crucial role in the progression and metastasis of CRC. Understanding the interactions between various types of cells in the TME can suggest innovative therapeutic strategies. Using single-cell RNA sequencing (scRNA-Seq) and clinical samples, fibroblast growth factor-19 (FGF19, rodent FGF15) is found to mediate a significant interaction between CRC cells and cancer-associated fibroblasts (CAFs), activating the hepatic stellate cells (HSCs)-to-CAFs differentiation.

Case Presentation: We present three cases of Javanese female patients with breast cancer (51 years old, 42 years old, and 55 years old) with liver metastases who underwent hepatic resection followed by systemic therapy.

Pancreatic cancer (PC) is one of the most malignant solid cancers, and PC metastasis, particularly liver metastasis, is a major cause of cancer mortality. A key event in tumor metastasis is the formation of pre-metastatic niche (PMN), which provides a microenvironment conducive to tumor cells colonization and progression. Various molecules loaded in tumor-derived exosomes (TDEs) contribute to PMN formation and distant tumor metastasis, by regulating immune and stromal cell function, inducing angiogenesis, and promoting metabolic reprogramming.
