The original description of the lower extremity bypass (LEB) provided surgeons with a reliable method of limb revascularization. The tenets of the operation have formed the foundation for the advances of surgical care. A careful evaluation of the chronic limb-threatening ischemia patient due to the numerous comorbid conditions is paramount to obtain the best possible outcomes. Use of all adjuncts including judicious target vessels control, completion imaging, and vein harvesting techniques to ensure optimal outcomes because a functioning LEB remains a key to successful limb salvage.

Case Discussion: A 29-year-old male with a history of corrected unilateral cryptorchidism presented with progressive bilateral lower extremity edema. Imaging revealed an inferior vena cava thrombus associated with a complex mass.

Cholesterol emboli syndrome is a rare syndrome of tissue ischemia and necrosis caused by the embolization of cholesterol crystals from atherosclerotic plaques, leading to vascular occlusion. This report documents a case of cholesterol emboli syndrome in a 72-year-old male with multiple cardiovascular risk factors including end-stage renal disease, atrial fibrillation, hypertension, hyperlipidemia, and type 2 diabetes. We describe this patient's atypical presentation with upper extremity rather than lower extremity digital ischemia as a presenting sign and significant subsequent functional decline exacerbated by his comorbidities.

Vascular neoplasms with epithelioid cytomorphology encompass a wide spectrum of benign and malignant lesions, including epithelioid hemangioma (EH), cutaneous epithelioid angiomatous nodule (CEAN), epithelioid hemangioendothelioma (EHE), and epithelioid angiosarcoma (EAS). Recently, the first case of a cutaneous hemangioma with epithelioid features harboring a TPM3::ALK fusion was reported. Herein, we report 4 additional cases, including 1 case with an alternate TPM4::ALK fusion, and expand on the clinicopathologic and molecular genetic features of these unusual vascular lesions.

Objective: To present, organize, and assess the methodological quality of the current research related to tDCS on motor function after a stroke and to identify gaps and clinical implications using an evidence mapping approach.
Methods: Six electronic databases (PubMed, Embase, Cochrane Library, Web of Science, CINAHL, PEDro), gray literature, and reference lists of articles were searched from inception until October 2023. The Assessment of Multiple Systematic Reviews 2 (AMSTAR-2) checklist and PEDro scale were used to assess the methodology quality of systematic reviews (SRs) and randomized controlled trials (RCTs).
Purpose: The aim of this study was to compare the efficacy, clinical outcomes, and complications of cable-asisted bone transport (CASt) and circular external fixator-assisted bone transport (CEFt) methods in the management of bone defects of the tibia.
Methods: A retrospective analysis was conducted on 32 patients who underwent segmental bone transport for tibial bone defects between January 2006 and January 2020 and met the study inclusion criteria. Patients were categorized into two groups: CASt group (n = 16) and CEFt group (n = 16).
