Purpose: This study aimed to determine the variations of the levator palpebrae superioris muscle and to reveal its morphological features.

Methods: This study conducted on 100 adult orbit cadavers in the Department of Anatomy, Istanbul University, used an exploratory, descriptive research design. The anatomical and morphological variations of the levator palpebrae superioris muscle and its relationship with the superior ophthalmic vein were evaluated.

Results: Variations of levator palpebrae superioris muscle were discovered in 11 of 100 orbits. Single (9%), double (1%), and triple (1%) accessory muscle slips were observed. The origin of accessory muscle slips showed variation as the accessory muscle slips originated either from the proximal or distal half of the levator palpebrae superioris muscle. Also, the insertions of accessory muscle slips were variable, as they were inserted into levator aponeurosis, trochlea, lacrimal gland, lateral orbital wall, or the fascia of the superior ophthalmic vein.

Conclusions: Accessory muscles associated with levator aponeurosis were found in a significant proportion of cadavers. These muscles may cause confusion in orbital surgery and should be taken into account during surgical planning and orientation in the superior orbit.

Pleomorphic adenoma of the lacrimal gland (PALG) is a benign neoplasm typically presenting with gradual, painless globe displacement and/or lid swelling. We report an atypical case of PALG in a 53-year old male presenting acutely, mimicking orbital cellulitis. Imaging demonstrated an extraconal rim-enhancing soft-tissue lesion medial to the left lacrimal gland, involving superior rectus and levator palpebrae superioris.

Upper and lower eyelid contour and positional changes after deep skin grafts in ablepharon macrostomia syndrome.

Orbit

January 2025

Department of Ophthalmology, Otorhinolaryngology and Head and Neck Surgery, School of Medicine of Ribeirão Preto, University of São Paulo, São Paulo, Brazil.

Ablepharon macrostomia syndrome is a rare congenital disorder caused by autosomal-dominant mutations. This condition is characterized by redundant skin, low-set ears, macrostomia, ambiguous genitalia, and underdevelopment of the both upper and lower eyelids. The shortening of the anterior lamella, septum and levator aponeurosis lead to a severe corneal exposure within the first hours of life.

Article Synopsis
  • Tarsal heights in Asian eyelids were found to be higher than previously reported, which may clarify issues with traditional double eyelid surgery methods.
  • A novel 'MAP method' for creating double eyelid creases was developed, involving specific suturing techniques that preserve important eyelid tissue and provide adjustable crease heights based on individual patient preferences.
  • This new technique offers better eyelid mechanics, a quicker recovery process, and longer-lasting results with minimal complications compared to classic procedures.

The Utility of Preoperative Phenylephrine Testing in Müller Muscle Conjunctival Resection Surgery for Involutional Ptosis.

Ophthalmic Plast Reconstr Surg

January 2025

Division of Orbital and Ophthalmic Plastic Surgery, Jules Stein Eye Institute, University of California, Los Angeles, California, U.S.A.

Purpose: Phenylephrine testing prior to Müller muscle conjunctival resection has traditionally been used to predict postoperative outcomes. The purpose of this study is to determine if preoperative phenylephrine testing impacts postoperative changes in eyelid position.

Methods: In this multicenter cross-sectional cohort study, 270 eyelids of participants with involutional ptosis and levator function >12 mm who underwent Müller muscle conjunctival resection were divided into 2 comparison groups.

Carotid-cavernous fistula (CCF) is a rare condition involving an abnormal communication between the carotid artery and the cavernous sinus. We present a unique case of posttraumatic unilateral CCF initially misdiagnosed as a corneal melt with iris prolapse and orbital cellulitis. The patient, a 25-year-old male, experienced swelling, bleeding, and sudden vision loss in the affected eye following a fall.
