Scrub typhus is an infectious disease caused by Orientia tsutsugamushi. It is transmitted through bite of chigger mite larvae and presents with symptoms such as fever, myalgia, headache, rash, and a characteristic eschar at the site of mite bites. This report details the case of a woman exhibiting acute febrile illness, bilateral pneumonia, and severe hypoxemia, prompting suspicion of scrub typhus due to the presence of a typical eschar on the pubic mound.

Purpose Of Review: To summarize current understanding of and recent literature on the management of sialorrhea in children.
Recent Findings: Sialorrhea is a symptom of oropharyngeal dysphagia and reduced clearance. Sialorrhea can be anterior, with forward overflow of saliva, causing skin rash, social embarrassment and spillage on communication devices; or posterior, where there is pharyngeal pooling of saliva, which may cause aspiration.

Background: Bier anemic spots, cyanosis with urticaria-like eruption (BASCULE) syndrome is a recently described entity with episodic urticarial lesions and white anemic halos on a background of erythrocyanosis, commonly affecting the lower extremities. Possible association with autonomic dysfunction remains poorly understood. Existing publications are limited, but the condition is suggested as highly underrecognized.

Our case report confirms adrenal insufficiency following three weeks of monkeypox infection in an immunocompetent patient. As there is no specific treatment for monkeypox, steroid replacement therapy remains the cornerstone of managing adrenal insufficiency. By highlighting the importance of early identification and tracing in our case report, we contribute to the growing body of knowledge on monkeypox and emphasize the importance of identifying adrenal insufficiency and other complications like meningoencephalitis, prompting a significant early intervention that can help manage these complications effectively, potentially improving patient outcomes.
