Actinomycetoma is chronic, suppurative, granulomatous infection caused by bacteria and requires prolonged antibiotic therapy preferrably in combinations. Nephrotoxicity is a common side effect of aminoglycosides used in the management of actinomycetoma. We report here two cases of actinomycetoma due to Nocardia species who received linezolid as a substitute to aminoglycosides after developing nephrotoxicity.

Introduction: Mycetoma is a chronic granulomatous infection, common throughout tropical regions, and is considered a neglected disease that mostly affects impoverished populations. Mycetoma is divided into eumycetoma, caused by fungi, and actinomycetoma, caused by filamentous bacteria. Clinical presentation is distinctive, and making the diagnosis is usually not difficult; however, access to safe and effective treatments is a major challenge.

Nocardial infections are rare but serious, often leading to systemic and cardiopulmonary complications. This is the first reported case of  causing constrictive pericarditis in an immunocompetent individual. We present a 37-year-old Caucasian female patient with no significant medical history who developed pericarditis symptoms after handling crates from China.

Actinomycetoma, a neglected tropical disease affecting the skin and soft tissues, is primarily caused by filamentous bacteria including Nocardia species. Here, we report a healthy 56-year-old man who has a one-year history of nodular lesions with seropurulent discharge on his right knee. Despite negative initial tissue culture, the sulfur granules that were partially acid-fast and Gram-positive branching filamentous rods were revealed in the tissue section.

Mycetoma is currently considered as a neglected tropical disease. The incidence of mycetoma is unknown but most of the worldwide cases are present in the "mycetoma belt" including countries like Mexico, India, Senegal, and others. The treatment of mycetoma depends on the etiological agent responsible for the case.
