Pancreatitis, panniculitis and polyarthritis syndrome (PPP syndrome) is a clinical syndrome with cutaneous panniculitis and polyarthritis on the basis of pancreatic diseases, which is quite rare. The occurrence of arthrosis and skin lesions is not completely synchronous with pancreatic diseases, so it is often missed or misdiagnosed due to the lack of obvious abdominal signs, with severe sequelae and high mortality. In this article, we report a case of PPP syndrome with severe rare osseous complications caused by chronic pancreatitis.

The pancreatitis, panniculitis, polyarthritis (PPP) syndrome involves the association of pancreatic pathology, panniculitis of pancreatic origin, and polyarthritis secondary to intra-articular fat necrosis. The incidence is unknown, and the mortality rate is as high as 24%. Treatment targets the underlying pancreatic pathology.

: Anti PD1/PD-L1 agents, including pembrolizumab, have revolutionized the oncological treatment of different types of cancer, including non-small cell lung cancer. The most frequent complications associated with this type of treatment are mild and are located at the thyroid, pulmonary or hepatic level. Sarcoid like reaction and mesenteric panniculitis secondary to pembrolizumab treatment are two very rare adverse effects.
