AI Article Synopsis

  • Pleomorphic adenoma (PA) is very rare in the subglottic region, as seen in a patient with dry cough and dyspnea, where a mass was found occluding 40% of the airway.
  • The patient underwent a successful transoral endoscopic CO laser microsurgery, allowing for the removal of the mass without needing a tracheostomy.
  • At a 2-year follow-up, there was no recurrence of PA, highlighting the effectiveness of this less invasive treatment method and the importance of examining the often-overlooked subglottic area.

Article Abstract

Pleomorphic adenoma (PA) is primarily found in the salivary glands, and is extremely rare in the subglottic region. Here we present a subglottic PA that presented with symptoms of dry cough and dyspnea. A submucosal mass was found in the subglottic region under laryngoscopy, occluding approximately 40% of the lumen. The patient underwent the transoral endoscopic CO laser microsurgery under high-frequency jet ventilation for mass resection, and the pathology report supported the diagnosis of PA. At the 2-year follow-up, there was no evidence of recurrence, and the patient is currently under regular long-term monitoring. Dyspnea and dry cough are nonspecific respiratory symptoms. When no findings discovered in the regular site, it should be noted that the subglottic area is often a blind spot for both pulmonologists and otolaryngologists, and as such, requires careful examination. Transoral endoscopic CO laser microsurgery under high-frequency jet ventilation was found to be an effective and less invasive method for treating subglottic PA. This approach helped avoid tracheostomy and resulted in better postoperative recovery.

Pleomorphic adenoma of the lacrimal gland (PALG) is a benign neoplasm typically presenting with gradual, painless globe displacement and/or lid swelling. We report an atypical case of PALG in a 53-year old male presenting acutely, mimicking orbital cellulitis. Imaging demonstrated an extraconal rim-enhancing soft-tissue lesion medial to the left lacrimal gland, involving superior rectus and levator palpebrae superioris.

A lipoblastoma is a benign tumor of adipocytes originating from embryonic white fat and occurs in the pediatric population. Congenital lipoblastomas, however, are rare, and the incidence of these tumors in neonates is unknown. Due to their rare presentation, congenital oral lipoblastomas can, firstly, pose diagnostic challenges for the pediatrician and must be differentiated from the more commonly seen oral lesions in the newborn and other rare malignant growths.

Although pleomorphic adenomas of the salivary glands are benign tumors, they can metastasize to distant organs without evidence of malignant transformation. We describe FDG PET/CT finding in a case of metastasizing pleomorphic adenoma in the right ilium occurring 22 years after initial surgical resection of a lip pleomorphic adenoma. On FDG PET/CT, the iliac metastasis appeared as an expansile osteolytic lesion with heterogeneous activity.
