Article Abstract

Purpose: To develop, train, and validate a multiview deep convolutional neural network (DeePSC) for the automated diagnosis of primary sclerosing cholangitis (PSC) on two-dimensional MR cholangiopancreatography (MRCP) images.

Materials And Methods: This retrospective study included two-dimensional MRCP datasets of 342 patients (45 years ± 14 [SD]; 207 male patients) with confirmed diagnosis of PSC and 264 controls (51 years ± 16; 150 male patients). MRCP images were separated into 3-T ( = 361) and 1.5-T ( = 398) datasets, of which 39 samples each were randomly chosen as unseen test sets. Additionally, 37 MRCP images obtained with a 3-T MRI scanner from a different manufacturer were included for external testing. A multiview convolutional neural network was developed, specialized in simultaneously processing the seven images taken at different rotational angles per MRCP examination. The final model, DeePSC, derived its classification per patient from the instance expressing the highest confidence in an ensemble of 20 individually trained multiview convolutional neural networks. Predictive performance on both test sets was compared with that of four licensed radiologists using the Welch test.

Results: DeePSC achieved an accuracy of 80.5% ± 1.3 (sensitivity, 80.0% ± 1.9; specificity, 81.1% ± 2.7) on the 3-T and 82.6% ± 3.0 (sensitivity, 83.6% ± 1.8; specificity, 80.0% ± 8.9) on the 1.5-T test set and scored even higher on the external test set (accuracy, 92.4% ± 1.1; sensitivity, 100.0% ± 0.0; specificity, 83.5% ± 2.4). DeePSC outperformed radiologists in average prediction accuracy by 5.5 ( = .34, 3 T) and 10.1 ( = .13, 1.5 T) percentage points.

Conclusion: Automated classification of PSC-compatible findings based on two-dimensional MRCP was achievable and demonstrated high accuracy on internal and external test sets. Background And Aims: Malignant biliary strictures (MBS) pose diagnostic and therapeutic challenges due to the frequent indeterminate results after initial sampling. Next generation sequencing (NGS) panel (BiliSeq) offers promise in MBS detection, but real-world performance remains uncertain. This study aimed to assess standard sampling techniques alone and with BiliSeq for malignancy detection in biliary strictures, and to evaluate management changes based on NGS.

Incidence of hepatobiliary malignancies in primary sclerosing cholangitis: systematic review and meta-analysis.

Clin Gastroenterol Hepatol

Department of Medicine, Yong Loo Lin School of Medicine, National University of Singapore, Singapore; Division of Gastroenterology and Hepatology, Department of Medicine, National University Health System, Singapore.

Background And Aims: Primary sclerosing cholangitis (PSC) is a known risk factor for hepatobiliary malignancies. We conducted a systematic review and meta-analysis of published studies to determine the incidence and risk factors for hepatobiliary malignancies in people with PSC.

Methods: Pubmed and Embase databases were searched from inception to April 10, 2024 for cohort studies reporting data on the incidence of cholangiocarcinoma (CCA), hepatocellular carcinoma (HCC), or gallbladder cancer (GBC) in PSC.

Primary chronic osteomyelitis (PCO) is a rare non-bacterial autoinflammatory severe disease that most often affects children aged 7 to 12 years and is much less common in adults. The most common areas of lesion are long tubular bones, however, any bones of the skeleton, including the lower jaw, can be affected. The clinical picture of this disease is complex and similar to many tumor and tumor-like bone lesions, which often leads to a significant delay in making a correct diagnosis.

Primary sclerosing cholangitis (PSC) is a risk factor for cholangiocarcinoma. When a child is diagnosed with both PSC and inflammatory bowel disease (IBD), evidence-based information on counseling families and risk management of developing cholangiocarcinoma is limited. In this case series (PubMed/collaborators), we included patients with PSC-IBD who developed cholangiocarcinoma and contacted authors to determine an event curve specifying the time between the second diagnosis (IBD or PSC) and a diagnosis of cholangiocarcinoma.

PPAR agonists for the treatment of cholestatic liver diseases: Over a decade of clinical progress.

Hepatol Commun

January 2025

Department of Biomedical and Pharmaceutical Sciences, College of Pharmacy, University of Rhode Island, Kingston, Rhode Island, USA.

Article Synopsis
  • Primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC) are liver diseases that damage bile ducts, leading to bile accumulation, liver injury, and increased risk of liver failure; current treatments are limited, especially for PSC.
  • Ursodeoxycholic acid is the first-line treatment for PBC, but many patients do not respond fully, indicating a need for better therapies; pruritus (itching) is a common and severe symptom due to cholestasis that is often inadequately treated.
  • The review examines the use of PPAR agonists, a type of medication that targets liver metabolism, as potential second-line treatments for PBC and PSC, highlighting recent FDA approvals for
