Background: Iliopsoas impingement (IPI) is an important complication after total hip arthroplasty (THA), and anterior cup protrusion is believed to be its main cause; however, the relationship between the hip center of rotation (COR) and symptomatic IPI or cup protrusion remains poorly understood. Therefore, the present study investigated these relationships.

Methods: The medical records of 138 patients who underwent unilateral primary THA were retrospectively reviewed. There were 8 patients (5.8%) who had symptomatic IPI. The COR and cup protrusion length measured with 2 methods were assessed on computed tomography. Risk factors for symptomatic IPI and the relationship between the COR and protrusion length were evaluated.

Results: Logistic regression analyses showed that anteroposterior position of the COR, sagittal cup protrusion length (SCPL) at the COR, and both axial and SCPLs at the most anterior margin of the cup were related to symptomatic IPI. Multivariable regression analyses showed that acetabular offset was related to axial protrusion length at the COR, and anteroposterior position of the COR was related to both axial and sagittal protrusion lengths at the most anterior margin of the cup.

Conclusion: Anterior position of the cup was related to symptomatic IPI and both axial and sagittal protrusion lengths at the most anterior margin of the cup. Anterior reaming and cup protrusion should be avoided as much as possible to prevent symptomatic IPI.

Introduction: Dual mobility (DM) total hip replacement (THR) features an acetabular component with an unconstrained tripolar build which ensures a greater range of motion without provoking any dislocation. We report three cases with extremes of surgical misadventure where we carried out a revision arthroplasty with a DM cup. The causes of the failure of the primary arthroplasty were multifactorial, even individually, and ranged from instability due to component malposition, trochanteric fracture, intrapelvic prosthesis protrusion, etc.

Article Synopsis
  • A rare, gram-negative, non-fermenting bacterium was identified as a potential pathogen linked to musculoskeletal infections, with a documented case found in an 80-year-old female after hip fracture surgery.
  • The patient suffered from tenderness and swelling at the surgical site six months post-operation, leading to the diagnosis of an osteosynthesis-associated infection (OAI) confirmed by microbiological tests.
  • Treatment involved a two-stage surgical revision and a two-month antibiotic regimen, resulting in the patient being infection-free and satisfied two years after the surgery.

Phagocytosis is an intensely physical process that depends on the mechanical properties of both the phagocytic cell and its chosen target. Here, we employed differentially deformable hydrogel microparticles to examine the role of cargo rigidity in the regulation of phagocytosis by macrophages. Whereas stiff cargos elicited canonical phagocytic cup formation and rapid engulfment, soft cargos induced an architecturally distinct response, characterized by filamentous actin protrusions at the center of the contact site, slower cup advancement, and frequent phagocytic stalling.

Association between cup fixation screw and iliopsoas impingement after total hip arthroplasty.

Clin Biomech (Bristol)

Department of Orthopaedical Surgery, Graduate School of Biomedical and Health Sciences, Hiroshima University, 1-2-3 Kasumi, Minami, Hiroshima, Japan.

Background: Iliopsoas impingement after total hip arthroplasty can result not only from acetabular cup but also from cup fixation screw. However, research addressing this screw impingement is scarce, leaving the details undetermined. This study aimed to elucidate the incidence and threshold of symptomatic iliopsoas impingement attributable to protrusion of the cup fixation screw into the iliopsoas muscle and to evaluate its impact on postoperative radiographic imaging findings and patient-reported outcome measures.

A Neglected Fracture of Acetabulum Treated With Total Hip Replacement.

Total hip replacement (THR) for osteoarthritis or inflammatory arthritis yields better outcomes than THR for patients with neglected acetabular fractures. The inferior clinical results mostly arise from an unforeseen bone deficit, making the treatment more time-consuming and complex for instances requiring acetabular restoration and bone grafting. There is a lack of research on the clinical results of THR in cases where acetabular fractures have been overlooked.
