As the adverse consequences of the industrial/modern agricultural framework, which encompasses high-input agrarian production and intensive cultivation, are increasing, an alternative is essential. Permaculture is a bunch of sustainable practices that incorporate an assortment of components and promote comprehensive and multi-polycultures including perennial plants, high degrees of biodiversity, crop-animal integration, whole watershed management, and self-sustaining on-site energy production, all of which straightforwardly affect the sustainable approach and promote ecological parameters. This case study attempts to better comprehend the local knowledge in terms of planning and fostering a permaculture system that considers their work, culture, and environmental concerns. In particular, this research focuses on the combined ideology, actual practices, and co-opting nature of three Nepalese permaculturists. The current study employs the notion of imaginaries to comprehend how permaculture may supplant the present agricultural system. Therefore, the study promotes and urges agricultural actioners to create profound and emotional associations with the planet, as well as their creativity and imagination, to impact good natural change.

Anagen effluvium is a rare adverse effect of methotrexate therapy, generally associated with high-dose regimens. We report a 12-year-old girl who developed abrupt hair loss 1 week after inadvertently taking methotrexate 15 mg daily for five consecutive days (total 75 mg), without other signs of systemic toxicity. Trichoscopic examination and trichogram findings confirmed methotrexate-induced anagen effluvium, and spontaneous hair regrowth occurred after 2 months.

Objectives: Naldemedine is a peripherally acting μ-opioid receptor antagonist used to treat opioid-induced constipation. As this drug does not cross the blood-brain barrier, it is believed that patients without brain metastases do not experience opioid withdrawal symptoms.
Methods: Here, we experienced a case in which a cancer patient without brain metastasis presented with anxiety and restlessness that was severe enough to interfere with daily life.

Situs inversus is a rare congenital anomaly that results in the transposition of the abdominal organs, leading to atypical clinical presentations, such as left-sided appendicitis. Acute appendicitis is the most common cause of right iliac fossa pain; however, its occurrence on the left side is exceedingly rare and often leads to diagnostic delays, which may result in serious complications such as peritonitis if not promptly addressed. Imaging modalities, particularly computed tomography (CT), play a critical role in the anatomical and pathological diagnosis, thereby guiding appropriate surgical management.
