Introduction: The changes occurring in the liver in cases of outflow deprivation have rarely been investigated, and no measurements of this phenomenon are available. This investigation explored outflow occlusion in a pig model using a hyperspectral camera.

Methods: Six pigs were enrolled. The right hepatic vein was clamped for 30 min. The oxygen saturation (StO%), deoxygenated hemoglobin level (de-Hb), near-infrared perfusion (NIR), and total hemoglobin index (THI) were investigated at different time points in four perfused lobes using a hyperspectral camera measuring light absorbance between 500 nm and 995 nm. Differences among lobes at different time points were estimated by mixed-effect linear regression.

Results: StO% decreased over time in the right lateral lobe (RLL, totally occluded) when compared to the left lateral (LLL, outflow preserved) and the right medial (RML, partially occluded) lobes ( < 0.05). De-Hb significantly increased after clamping in RLL when compared to RML and LLL ( < 0.05). RML was further analyzed considering the right portion (totally occluded) and the left portion of the lobe (with an autonomous draining vein). StO% decreased and de-Hb increased more smoothly when compared to the totally occluded RLL ( < 0.05).

Conclusions: The variations of StO% and deoxy-Hb could be considered good markers of venous liver congestion.

Background: The diagnosis of acute myocardial infarction is straightforward when it presents with typical symptoms. However, some patients can be asymptomatic, and some can present with atypical symptoms.

Case Presentation: We present a very rare case of acute myocardial infraction that presented with headache and right upper limb numbness of 2-day duration in a 61-year-old female Middle Eastern patient.

Article Synopsis
  • The study focuses on acute coronary syndrome (ACS), specifically examining the angiographic characteristics of patients with Non-ST-segment elevation myocardial infarction (NSTEMI) undergoing coronary angiography.
  • Conducted at Lady Reading Hospital Peshawar, the research involved 300 patients, primarily middle-aged men, revealing that 73% had significant coronary artery disease (CAD), with many showing multivessel involvement.
  • The findings suggest that NSTEMI patients commonly present with multiple risk factors and often exhibit severe and multivessel CAD.

Background: Tip detection-antegrade dissection and re-entry (TD-ADR) technique allows operators to accurately observe both guidewire tip direction and a true lumen in chronic total occlusion (CTO) lesions, while the torque direction of the guidewire on IVUS images does not invariably correspond to that on fluoroscopic images.

Case Summary: A 41-year-old man with hypertension who smokes presented with sudden onset of dyspnoea, acute heart failure, and ischaemic findings on electrocardiogram; we performed percutaneous coronary intervention (PCI) for a sub-totally occluded mid-left anterior descending artery lesion. All antegrade wiring attempts failed to enter the distal true lumen followed by subintimal tracking and re-entry technique.

Refractory type 1 plastic bronchitis in a child; case report.

BMC Pediatr

July 2024

Department of Pediatrics, Faculty of Medicine, Tanta University, Tanta, Egypt.

Background: Plastic bronchitis (PB) is a rare pediatric pulmonary condition characterized by the production of branching bronchial casts that cause partial or total obstruction of the bronchial lumen.

Case Presentation: We describe a 13-year-old boy with a history of bronchial asthma and left lower lobectomy, with persistent cough and left-sided chest pain when he went to the emergency room. Chest radiography showed complete left lung opacity denoting total left lung collapse, and flexible bronchoscopy revealed cohesive casts totally occluding the left bronchus, with frequent recurrence that finally ended with left pneumonectomy.

Adhesive dentistry has made it feasible to design restorations with high aesthetic qualities and little invasiveness. However, the freshly cut dentin after the tooth preparation needs to be sealed to prevent its contaminations, bacterial leakage, and hypersensitivity. Consequently, the immediate dentin sealing (IDS) method has been advised.
